Fiat iustitia, et pereat mundus.Well, that may seem extra shocking because you haven't gotten the memo that the Gotei 13 of before 1000 years ago, the period from which she and Yamamoto hails from, were pretty much all Blood Knights and Psychos For Hire like the modern 11th Division (and perhaps Mayuri, for the less combat-oriented types)... and that she's the very first holder of the title of "Kenpachi".
No, I do not kid. Her real/birth/original name is Yachiru Unohana, BTW.
PS: Also, the anime ends right before the current manga arc, so none of this will be explained in the anime. Kubo probably pressured the studio into waiting until he finishes his entire arc, to avoid them accidentally screwing it up with the wrong kind of padding/filler or something.

He used to be even more ruthless before he properly (re)formed the Gotei 13 one thousand years ago into the current, orderly organization that we know. The arc's Big Bad notes that Yamamoto and the Gotei 13 had gotten quite soft since those days.

Back then, Hell used to be the nice play where to go.
Maybe he is, given that the only depiction of Hell that we know of is the Hell Chapter movie, and the only thing that's agreed to be canonical about it is the World-Building aspect... sort of. I'm not sure myself.
