  • Prop Recycling: The spears the Mortaja enemies throw at you use the same sprite as the ones used by the Tres Angustias boss from the first game.
  • Refitted for Sequel: Per the artbook and developer diaries videos:
    • Lesmes, Incorrupt Sacristan was originally designed for the first game but his boss fight was repurposed into Quirce, Returned by the Flames. Along with Susona, he is a boss idea that got reused and expanded for the sequel. Fittingly, he reuses some of Quirce's effects, especially the twisting fire columns.
    • Svsona, Fermosa Fembra was originally designed for the first game's DLC but was never used. Like Lesmes she is a repurposed boss in the sequel.
    • The Elevated Temples area is based on an unused concept for the Archcathedral Rooftops area section from the first game.
    • The developers wanted to include an area similar to Two Moons since the first game but couldn't find any spot where it would make sense and instead back then ended up using the idea in the 8-bit Game Within a Game, "Blasphemous: Alcazar of Grief".
    • A Double Jump ability for the Penitent One was considered for the first game, but was decided as too much of a Game-Breaker. It was balanced out and implemented for this sequel.
  • What Could Have Been:
    • There's unused concept art for an area called "The Thorned Monastery", which depicts a church covered in brambles. One of the quest items even references it in its lore description.
    • There were going to be mechanical devices that required power from hitting them with Sarmiento and Centella's true dexterity electric charge. Still, the developers realized that this required the player to build the charge and avoid being hit until they reached the device, which they deemed too punishing. So, instead, they implemented the teleporting mirrors as the navigation mechanic for those weapons.
