Lyuba is happy with no longer needing to keep her relationship with Kupitman in secret, but Kupitman (who's still recovering from heart attack) thinks that under all those smiles, everyone secretly mocks them. Especially Bykov. When Bykov actually visits him, Kupitman assumes that it's the start of a war, not realising that Bykov was there to congratulate him on sticking up with Lyuba, in his delusions seeing hidden poison in his every word — which he immediately reveals to Lyuba: in his current condition, he's too pathetic to even mock, so their "enemies" would concentrate on Lyuba alone. Meanwhile, Kisegach, after what happened with Kupitman, starts worrying that the same may happen to Bykov (who is about the same age), and tries to force him into complex examination: she wants to be absolutely sure that he is perfectly healthy and has no risks. Bykov doesn't consider Kupitman's example to be "an example" — after all, he is an alcoholic, who nearly killed himself by viagra, and this is certainly not something which may happen to Bykov. When she insists, the laboratory assistants mistakes Alexei giving them Kisegach's request for a prank, and puts in every single disease they can think of, making it impossible to tell the truth from lies. Alexei just throws the list away, but misses the bucket, resulting in Lyuba finding it and giving to Kisegach, who goes to confront Bykov about it. And Bykov, after explaining everything, uses intercom to mock them all for their stupidity — only for Kupitman to hear it and go out of his ward, thinking that they're laughing at him and Lyuba, and start calling him out on it. He knows that they're weird and funny couple, but this is their decision, so show at least some respect! Lyuba gets offended by that "weird couple" comment, and now doubts whether they even should stay together at all.

Polina's patient shows attraction to Timur. She feels slightly awkward after learning that Polina is Timur's ex, but Polina assures her that there's nothing wrong with it and she doesn't care whom Timur would date, so go ahead. Timur uses "I'm married" excuse to get rid of the girl who tries to hit on him, only to learn from her that Polina isn't against him dating other people; but exact wording tells Timur the real meaning: Polina no longer cares about him, at all. Gleb tries to assure him that Polina acts this way because she knows that Timur is always nearby and under her control, so she never risks losing him permanently. Timur has to pretend that he started losing attraction to her, and she would soon start panicking. Timur apologises before his patient (who introduces herself as Masha), and invites her on a date. Polina allows Masha to leave, without any questions, and now Timur doubts whether plan actually works, but Gleb assures him that this is only the beginning: he didn't do anything yet, that's why Polina isn't jealous yet. He should start by going on that date. On a date, Timur tries to call Gleb (who should answer loudly enough to provoke Polina's jealousy); this fails to produce any results, but Timur insists on continuing. He invites Masha to his house, and asks Masha to inform Polina that she would be absent for a whole night; if this wouldn't provide any reaction — nothing would. But, to Timur's disappointment, Polina doesn't react to it, either. Only at the very last moment before leaving, Gleb notices that she's nearly wasted: instead of showing her jealousy, she tried to suppress it with alcohol; now, she hates Timur even more, as, after all the claims that he still loves her, he just jumped at another chick. Gleb, realising what's going on, calls Timur and warns him that this is his only chance: he must rush to Polina, and try to improve relationship with her. When Timur arrives, Polina still looks angry, but Timur manages to act like a gentleman — covering her with a blanket, giving her some medicine to compensate for alcohol's side effects, and just act like he cares about her — which seemingly serves as the first step on the path to improve their soiled relationship: Polina falls asleep with the smile on her face.

Alexei's new patient is... Bykov, A.E. This is not that Bykov, just the guy with same surname and initials, which Lyosha considers to be funny — but Doctor Bykov feels annoyed by this, and kicks him out of on-call room. Then Alexei notices that his patient shares not only surname and initials of his boss, but also his speech manner, which he immediately tells to the patient. The patient then asks whether his boss is called Andrey. When answered, the patient reveals that he is Bykov's cousin, Anatoly; he knows that he a has cousin in Moscow, a doctor, and arrived here to meet him. He asks him to tell Andrey certain words, which should remind him about their first meeting. But Bykov refuses to believe that this guy may be his cousin; he's not the first and not even the second smartass to try that tactic to get free VIP conditions. When Alexei returns to take Anatoly for blood test, Anatoly starts mentioning exactly those things which Andrey expected him to want — like VIP ward. Alexei realises that his boss was right, and accuses Anatoly of lying. As another attempt, Anatoly offers to tell him a couple of stories from his childhood, and ask his "cousin" whether those really happened. But every such "story" is too generic, being rather typical to the time period when they were children: everyone were like this back then. But then he tells the last story, which actually convinces Andrey to visit his alleged "cousin" — the one which involves huge she-cook from a pioneer camp. After seeing Anatoly, Andrey admits that they are really cousins. Two Bykovs drinks together, discussing their past... but then Andrey notices inconsistence between their memories: while both stories involved female cook, Andrey put a pan of hot sauce on her (he mistakenly thought that it was cold), while Anatoly had lost virginity with her. They quickly notices more problems like this, and then Anatoly's aunt calls and reveals that his cousin is indeed called Andrey... but he's not a doctor at all, he's a former orderly, now taxi driver.

This episode provides examples of:

  • Internal Reveal: The exact moment when Kupitman and Lyuba's Secret Relationship stops being secret, since, thanks to Bykov, now literally everyone is aware (and if someone is not, Kupitman then announces this over intercom anyway). Their main plot shifts to how everyone reacts to this reveal.
  • Long-Lost Relative: Subverted. After everything pointing to that Anatoly Bykov and Andrey Bykov are cousins, turns out that Anatoly's cousin isn't a doctor, as he previously thought; he's a former orderly, now taxi driver. Anatoly wasn't aware of this himself until he was informed.
  • Open Mouth, Insert Foot: In his attempt to defend himself and Lyuba, Kupitman accidentally offends Lyuba, not only pointing that they are a "weird couple", but also calling her "lone, ageing woman", pressing two of her weak points at once. She dumps him after that.
  • Operation: Jealousy: Timur's plot in nutshell; he tries to awake Polina's "sense of the ownership" by starting dating other woman. Instead of intended result, this nearly causes her to hate him more, and he barely manages to salvage the situation when he stops acting like an asshole and acts nicely to Polina.
  • The Paranoiac: Kupitman. While still recovering from his heart attack, Kupitman sees lies under any smile, hidden insults under any compliment (whether related to his relationship with Lyuba or not), and is dead sure that everyone are conspiring against him and Lyuba. He doesn't believe Lyuba, either, when she insists that everything is fine. And he doesn't even try to hide this from anyone, rudely accusing them of conspiring against him. In fact, even Bykov was entirely sincere on congratulating him, not to mention everyone else whom he accused of treachery. In his final attempt to "defend" Lyuba, he only insults her. By the end, he never owns his mistakes.
  • Shaming the Mob: Kupitman tries to shame everyone into treating him and Lyuba seriously (he thought that they're laughing at them, while they discussed something entirely unrelated). This backfires spectacularly, due to his choice of words.
  • Ship Tease: First signs that Timur actually has a chance to mend his relationship with Polina, since when he starts actually acting nice to her instead of just harassing her all the day, she likes it.
  • Too Dumb to Live: When Kisegach tries to use Kupitman's heart attack from the previous episode (he still recovers after that) as argument in favour of complex examination (to not repeat his fate), Bykov points out that Kupitman is a bad example, because his problems were caused only by his own stupidity and absolutely idiotic decision to enchant already barely alive body with viagra.
