JD dresses like a clown to fill into Elliot's shoes. Dr. Cox tries to have the hot Dr. Miller on his side in the war with Kelso. It's Turk's turn to take the narration -since JD is a clown and has nothing to say- and he is afraid of sending the invitations that mean that his wedding is finalized and there is no going back. But Carla makes him know that she is the right person to be with.
- Actually Pretty Funny: Carla and JD crack up over Kelso's "Oh no he didn't".
- Foreshadowing: The episode ends with Cox staring at Dr. Miller's chest during their last conversation then her ass as she walks away. His crush will continue to develop later.
- Happily Married: Eliot's patient has a short, frumpy husband, but they're passionate for each other.
- Has a Type: Cox is once again falling for a strong woman who doesn't take his shit.
- Oh, Crap!: Turk damages his patient's arm during surgery. He doubles down on the crap when he acknowledges that he just did permanent damage to the dexterity of a concert pianist.
