- Bob develops a nastier personality every time he's sick/injured, even if it's just a minor ailment such as the common cold.
- Bob was formerly an All-Loving Hero, but he becomes outright physically aggressive because of his ailment.

- Bob becomes a little less concerned about what is and what is not appropriate behavior, but doesn't become an outright Jerkass.
- Bob has the tendency to be more whiny and demanding because of his ailment, but doesn't do anything outright abusive.

- Bob is in pain or is otherwise extremely miserable because of his ailment, and lashes out at people because of this.
- Bob's ailment affects his mental capacities (such as a brain injury, or Alzheimer's disease/dementia), and causes him to not realize what kinds of behavior are inappropriate.
- Inverted: Bob becomes a much nicer person as the result of his ailment.
