  • The main premise of the movie, in which an Eldritch Abomination grants a human some addictive substance which causes him to hallucinate strange flashing colors by forcing him to make contact with another human just to eat their brains is already creepy enough.
    • It gets worse if you compare Aylmer to that of a drug, more specifically ice.
  • Speaking of which, Aylmer, just Aylmer, is enough to make you deeply horrified, of the fact that not only does he eat brains for a living, but the fact that he can make anyone experience unsettling visions due to withdrawal, and the fact that he's blue makes him more reminiscent of Crystal Meth.
  • Whenever Aylmer attacks someone, especially the night watchman, and a hooker, the film's music suddenly turns into an unsettling alien-like version of the main theme, complete with drums and violins.
  • While Brian tries to get rid of Aylmer's juice as part of his withdrawal he slowly begins to lose his sanity as he vomits and lies down on the floor unable to do anything, then all of a sudden, as he tries to pull something out from his ear, he pulls out a long string complete with blood, Brian's scream makes it even worse.
    • When Brian does manage to "pull it out", all of a sudden his insides come out of his ear, thankfully, it was only a nightmare, but still.
  • The ending, after successfully grabbing Aylmer and trying to kill Mr. Ackerman, the latter somehow is alive enough to get up and forcefully grab Aylmer from Brian as he tries to give him his fluid, which results in him giving Brian too much fluid, as Mr. Ackerman forcefully strangles him, we get a nice shot of Aylmer bleeding and screaming in agony while his inside mouth is visible.
    • You thought that was bad? It gets worse, as Mr. Ackerman dies, Brian gets an overload, which causes him to grow a lump on his forehead while blue fluid comes out of his mouth, and as he screams himself into his apartment, he tries to shoot himself in the head to put himself out of his misery, but instead, it goes all downhill from there.
      • As Brian locked the door, with the help of the police officers the apartment owner called, Brian's brother and the officers try to break down the door, only to see light emitting straight out of Brian's now visible head, and what makes this scene more disturbing is that the camera just pans above Brian as electricity cackles all over before the film abruptly ends, leaving the film with No Ending.
