** Patients, much like the Abominations, will ram you constantly and even can bypass your TAC-Bar, making them hard to avoid. Making matters worse is that in the Seeking Asylum chapter, you face ''hordes'' of them, and in confined areas to boot. They're usually GoddamnedBats in Arena Mode, however, due to being a late-game enemy encountered in the Murder Room, in which the spacious area at least allows you to avoid them better, but their ability to charge right at you combined with the various traps can make them annoying.
* GoddamnedBats: The Needle-wielding Harmacists go down like any other enemy in the game, but their ability to slow down players with their needles and [[InterfaceScrew make the screen suddenly daze colors]] can throw off some players.

* DemonicSpiders:
** Nexus Core Riot Guards are a massive pain to deal with. To start, their shields can absorb just about any attack short of explosives and it'll block any shots if they are in front of other enemies leading to you potentially wasting your ammo. The only way to deal with them normally is to attack when they are raising their shock baton to strike but not only is this window fairly small, even then their healthpool is massive as you need to peel off their armor first to the point they can take multiple Sweet Spot shots or heavy melee strikes to fall. Compounding that is the fact their own attacks deal high damage and come out fairly fast which can destroy your Corpus block surprisingly quickly especially if you are swarmed by them which is the most likely case as they spawn semi-frequently.

* NightmareFuel: Seeing a lot of the character designs in full 3D in all their gritty, blood-soaked glory can be disheartening on its own, but special mention goes to the loading screen portraits of all the primary characters. Lit only by a passing red light, they're all very realistically detailed renditions of the main sextet of characters... and then you get to Hank, who is ''not'' wearing his mask, and so you get to see his mangled, jawless face in full detail framing those piercing red goggles, revealing that he's also missing his nose.

* SugarWiki/FunnyMoments: Madness was never exactly serious, so there's a couple well-placed jokes.
** Once voice lines for multiple named characters were uncovered, it was found that the Sheriff's death rattles, presumably as improv by his voice actor, include a full recreation of the "Howie Long Scream" StockScream.
** Early into the game, Hank needs to unlock a security door. You initially get a prompt to ...Hack? a computer. Engaging with it has Hank scratch his head for a second and admit he doesn't really "do" hacking. He does, however, find a sledgehammer place conveniently close to the mechanism powering the lock.
** The public demo involves two missions leading up to the confrontation of The Sheriff. When you complete the second one, Sanford goes to plant the demolitions and... there is no effect on the door. The following conversation becomes a huge, progressively less subtle LampshadeHanging on the limited scope of a game demo.
** When Hank and Sanford assault a barracks, they pass by an Attendant who quickly shutters his post and refuses to come out. You can't hurt him, but thanks to the "Ring for Service" bell on your side of the shutters, you can annoy him to no end.
