Further theorizing why the Doctor didn't show up in Children of Earth: he was trying to convince whatever intraplanetary goverment exists on the 456's homeworld that their drug epidemic was indeed a major issue and not something to scoff about. Basically, in a major difference from most Whoniverse species the 456 - despite their technological advancements - are, culturally, extremely similar to humans. Complete with their own powers that be refusing to properly acknowledge a dangerous and probably deadly drug epidemic sweeping their planet. The "ambassador" we met is either a tweaker or the equivalent of a drug cartel boss from the 456 homeworld, and the Doctor was trying to show his government how terrible the consequences would be of their own ignorance of the issue. This would also explain why exactly he didn't come to Earth's aid, he was trying to enforce a more systemic solution to the problem, since probably Earth isn't even the only planet the 456 have exploited for their gain.

Torchwood Four is a catch all term for those not affiliated with the actual institute, but still dedicate their lives to protecting humans from supernatural threats.
