One location seriously guilty of this trope is the Phrik Mine, which is run by snooty administrator droid F-TO (Pronounced "Feet-Oh"). For starters, the elevator accessing the mine proper is a simple platform that someone could easily fall off of while it's moving, and after it gets there, it opens ''right next'' to an open pit; then there's the Laser-Powered Fusion Reactor, which has no handrails on its catwalks that span over an equally deep pit.
