* WhatCouldHaveBeen:
** An early iteration of the script would have established Ezekiel Banks as being related to David Tapp, one of the main characters of the [[Film/SawI first film]]. However, the writers dropped the idea because it "didn't pass the smell test".
** Maybe not for this film, but during its development, it was discussed about possibly bringing back Mark Hoffman, who was last seen being left to his death in ''Film/Saw3D'', in a future film.
** Although the film is the first in the series to not include John Kramer or Billy the Puppet (despite that the title referencing the spirals from Billy's cheeks), the crew widely discussed bringing Creator/TobinBell back until the last day of filming, although John was never included in any version of the screenplay. Among the ideas considered:
*** John Stolberg and Pete Goldfinger proposed including a [[TheStinger post-credits scene]] in which John met a young William Schenk after his father's death, bonding with him and gifting him with Mr. Snuggles, but the idea was discarded.

** For Mr. Snuggles' voice, to differentiate from Tobin Bell's voice for Billy the Puppet, numerous voices of women, children and men were considered before the crew settled on a computer simulated voice two days before the sound mix finished. The movie was shot primarily in Toronto, Ontario, with the area underneath the Gardiner Expressway used as a major set piece, and older-model streetcars visible in the window of the butcher shop.
