'''Founding of the League'''. A group of foreign constables chases a beast to Yharnam and perishes fighting it. Its SoleSurvivor, Valtr, devours the beast's corpse, gaining the ability to perceive [[FantasticVermin Vermin]] in human blood, and starts the League of Confederates to discover and to crush all of it.

'''Final Hunt'''. The Blood Moon rises again, but is held back by Rom. Another outbreak of the Scourge occurs anyway and prompts the current Vicar of the Healing Church, Amelia, to call for another Hunt. Father Gascoigne heeds the call and sets out to hunt again, but succumbs to the Scourge and kills Viola as she tries to bring him to his senses. The Upper Cathedral Ward falls to the beasts, prompting the remaining Choir researchers to evacuate, including Yurie the Last Scholar (who heads for Byrgenwerth) and False Iosefka (who takes with her the Orphan of Kos' Cord of the Eye, which the Choir had previously retrieved from Willem, and heads for Central Yharnam). A [[PlayerCharacter sick person from a foreign land]] stumbles into Iosefka's Clinic and receives Blood Ministration, before being drawn into the Hunter's Dream.

'''Blood Moon''': Triggered by defeating Rom and approaching the mysterious woman in a BloodSplatteredWeddingDress. Everything that Rom had hidden is now visible, including the Blood Moon and Lesser Amygdala. Several survivors, if they've made it this far, will go insane. Eileen's questline ends with the Bloody Crow of Cainhurst fight. The Nightmare of Mensis is accessible. After defeating Mergo's Wet Nurse and returning to the Hunter's Dream, Gehrman makes his offer to sever the Hunter from the Dream, or not. This choice determines what phase occurs next.

First is the Hunter's Nightmare proper, where the souls of hunters who died when blood-drunk are trapped in an endless bloody hunt for beasts. Second is the Research Hall/Astral Clocktower, where the remnants of the Healing Church's early experiments to create Celestial Mobs live. At the top of the Clocktower sits Lady Maria, who was trapped in the Nightmare with the rest of the Old Hunters. After defeating her, the Hunter progresses to the Nightmare version of the Fishing Hamlet, where they find the Orphan of Kos, born properly in the Nightmare. Once he and his shadow, the "Sweet Child of Kos", are slain, the nightmare is dissolved.

At some point after the Great Schism, several important events take place at Byrgenwerth, though it is unclear when exactly and in which order they occur: Rom the Vacuous Spider[[note]]commonly believed to have been a scholar before she was, according to Micolash, granted eyes-on-the-inside by Kos(m) and ascended to Kin of the Cosmos[[/note]] is placed under the lake near Byrgenwerth to hold back the Blood Moon and, more generally, to protect the minds of Yharnamites from the eldritch truths unraveled by the scholars' research. Master Willem seeks to line his brain with eyes -- and [[GoneHorriblyRight succeeds in a way]] that leaves his body a [[BodyHorror deformed, unintelligible husk]] in the Waking World; his students blame Rom for this.[[note]]"The Byrgenwerth spider hides all manner of rituals, and keeps our lost master from us."[[/note]] The Byrgenwerth Lecture Hall is ripped from the Waking World and transported into the Nightmare. The Byrgenwerth College and the [[TheLostWoods woods]] around it are declared [[ForbiddenZone forbidden ground]] by the Church.
