* Dean, Sam, and Jack's reactions to finding out they're the only living things left on Earth are one of disbelief and hopelessness. Even worse, when they try to bargain with Chuck that they will go along with his story, he spitefully states that he will simply leave them alone on an empty world, spending the last moments in shame.
* When Dean finds a dog Chuck missed, he's overjoyed and names the dog Miracle, clearly intending to adopt the dog. Then Chuck semi-literally [[KickTheDog kicks the dog]] by disintegrating Miracle to deny Dean even that bit of happiness.
* In a way, Michael's death. * Literately, everything about Cain is this. After killing all those demons, as he had already expected, he fell back under the Mark's influence and started killing his descendants because he knew extinguishing them would help him make up for having committed the first murder and starting the very idea of murder in the first place.
** Also, his relationship with Dean. Cain genuinely feels killing Dean would be saving him from his fate as he knew the Mark would inevitably transform him into the same monster he is now. Compare that to how he used to be resistant to the Mark and anyone would feel sorry about him.

Not only is it a grim reminder of the fate that seems so inevitable, but it also reminds him of how Cain is effectively what he would be like if he killed Sam and just gave up. And this affects the survivors' mental state. One angel has fallen on hard times enough to [[DrowningMySorrows become a drunkard]] (note that angels are far more resistant to alcohol). Others silently allow Lucifer to lead them (not that he does a great job at it). Even Naomi, the same [[ControlFreak controlling]] and power-hungry monster from Season 8, has become so defeated that she resigns herself to fate while telling Castiel to find any help he could. Victim 3, Mr. Caldwell, was attacked in the dead of night, awoken from a sound sleep to be dragged away in a sack, struggling and screaming for help while his son Jimmy, who can't be any older than six or seven and thought he was getting an early visit from Santa, watches thinking Santa is doing this horrible thing to his family. And to make matters worse, these all happens within days of Christmas, which means a holiday that's supposed to be about love, joy and family is being spent in mourning and not feeling safe in their own homes. The only possible solace is if the person Sam and Dean found struggling in that sack in the MOW's basement was Jimmy Caldwell's father, then they were probably able to rescue him after killing the monsters of the week and reunite him with his family, giving at least Jimmy a hope for emotional recovery.

* When Mary's spirit appears to her children, all she can say to Sam is "I'm sorry." When Sam, with tears in his eyes, asks for what, all Mary can do is look at him sadly. Considering what we learn in season 4, it's retroactively heartbreaking.

Apparently, she and an abusive ex got into a pretty bad fight, culminating with Charlie breaking up with him and the boy stating that if she walked out on him, he would commit suicide. An enraged Charlie then told him to do it and left without looking back. Needless to say, her ex was a man of his word, and, well ...
** For all her snark at the beginning of the episode, it's pretty clear that the secret has been weighing on her for some time now, even if the whole thing had never been her fault in the first place.



* Just the concept of the entire episode. Your loved one comes home from a long day at work and you greet them happily, only to suddenly find yourself being beaten within an inch of your life. If the shapeshifter had left any of its victims alive, not only would it have soured their relationships forever, they would've left the abused party with a ''ton'' of PTSD and trauma as well.
