** Eventually, the player will stumble into an abandoned GL Labs facility, which had been underneath the dollhouse(and by extension the rest of the mansion) all along. There even is [[SchmuckBait a tram you can take to reach the surface]], though doing so before you finish your goal is not advised. Then you find the source of how this all came to be: a HellGate that, due to a botched ritual, has been leaking evil spirits into the house, with many of them becoming the specimens you have been running from. While a ritual to close the gate has already been initiated, you still must retrieve the aforementioned candles. Unfortunately, the second step of the ritual requires a spirit to be bound to a crystal, which translates to having to choose which of 3 specimens(6, 8, or 4[[labelnote:*]]Original version only[[/labelnote]]/11[[labelnote:*]]HD version only[[/labelnote]]) you want to be chased by for an extended sequence until you can lure them into the sealing chamber. Each specimen gets a couple of new gimmicks to make it easier for them to catch up to you, and at this point you no longer have the doll to heal you.

Even better, they also gathered new quotes upon killing you.

** Afterwards, you have to make it back to the tram in a limited amount of time. If you do not get it on time, you get to see a ''graphic'' picture of your character [[KillItWithFire getting incinerated]]. And what's even better, you can see Spooper (see on that below) guarding the tram. You will need to strike it with your weapon to escape the facility.

* Probably the creepiest thing about this hospital is that it is so unknown. From what the notes mention, it seems that the people there were having blackouts, waking up to find they had done things to harm and kill others. This seems to be some sort of Demonic Possession, as described by a nurse who felt as if something was trying to control her and the only way she could stop them was dousing herself in radiation. Oh, and the ending implies that there is more beyond the hospital that is to be seen.

** Although the original's Endless Mode and HD Renovation has the monster become ''much'' more faster.

** The second one is a seemingly innocent minigame - you find a small Gameboy-like device in the basement, which seems to have a DatingSim game of sorts. At first it seems like a fairly typical dating sim, but then the dialogue options start becoming either much darker ("Everything is disgusting here", "It's like my whole body", "is trying to peel apart on the inside") or completely cryptic ("The flies are, can't fit any more", "My home is inside you")... And then, on the third day, the girl you were talking to in the minigame suddenly becomes a zombie of some sort, with a completely dull look on her face. (Before that, she remarks she's not feeling well, implying that she is probably suffering from some unknown and possibly terminal illness), and on day four, you are pretty much forced into an endless dialogue loop (well, almost endless, as you can always exit the minigame, but if you decide to keep going, you will be stuck there for a while), where the girl's face slowly disappears as she keeps talking.
*** It doesn't stop in one go, actually - you can choose to replay the game, and it's all but implied that with each time, the girl becomes more and more aware of being stuck in a GroundhogDayLoop. So the only way to get her and the protagonist out of this loop is to replay the game over and over, getting new dialogue options (which become progressively creepier, too). Finally, you get the FailureIsTheOnlyOption ending, where the girl reports you and you are both transferred out of the school. The whole thing is a total MindScrew in and of itself, to say the least.

* Even to those who are not claustrophobic, the build-up to Specimen 10 can be pretty terrifying. You enter a dark sci-fi-looking area that wouldn't look out of place in ''VideoGame/DeadSpace'' or ''Film/{{Alien}}''. While you do manage to turn the lights back on, they go back out not long after, and the only way forward is a vent with dried blood on the floor. When you enter the vent, you may notice sounds while you're crawling through the vent and walls of goop that you have to cut through. And when you finally leave the vent and turn on the power switch, you meet with the above-mentioned Specimen.

* [[https://soundcloud.com/akuma-kira/sets/spookys-ost The soundtrack]] has quite a good amount of rather disturbing tracks. To name just a few...
** "Amputated Marshmallow", which plays when you get trapped in the endless hallway...
** ...and then "[[HellIsThatNoise Sour Lemon]]", which plays immediately after Specimen 9 catches you.
** "Strange Wiggles" does sound somewhat demented, but not as demented as...
** ..."Leave your shoes, leave ALL your shoes" which sounds like corrupted circus music mixed with some really disturbing noises.
** "Your Consenting Mind" sounds like something out of an old horror movie.
** "Unknown Hug", which is pretty much "OhCrap" in the form of a music track, especially given the fact that it plays just when you encounter the very first monster in the game.
** The same "oh crap" feeling (although to a lesser effect) also applies to "Writhing Sneezes", which plays when you are being chased by either Specimens 3 and 10.
** "[[HellIsThatNoise Lusting Strawberry]]". Which sounds like something from ''Franchise/SilentHill''. Makes sense, considering this is Specimen 5's chase theme, and Specimen 5 is obviously inspired by Silent Hill...
** "The Merchant" -- Specimen 6's chase theme -- sounds like something straight out of ''[[Videogame/TheLegendOfZeldaMajorasMask Majora's Mask]]''. Again, makes sense; Specimen 6 bears quite the resemblance to the Happy Mask Salesman.
** "OUO", the theme that plays in Specimen 6's intro area. It sounds like a really creepy tribal-sounding theme with ominous vocalizations, and wouldn't sound out of place in ''Majora's Mask'' either.
** "We Have The Beef", despite having quite a silly name, is hands down one of the most disturbing tracks in the game.
** "Getting There", the chase theme for the new Specimen 10, is the first chase theme in the entire game to sound outright depressing. As well as having some truly alien noises.
** "Gummy Worms with Gummy Bear Heads" sounds incredibly ominous and seems to be building up to something big, before going back to being low.
** "Here Comes Trouble" sounds like a chase theme from ''VideoGame/ClockTower''.
** "Something in the Water" sounds like if thalassophobia, fear of the the sea or fear of being in the ocean, had a theme.
** "Fried Calamari" is unsettling for a slightly different reason: for being so much more bombastic than most of the other tracks in the soundtrack, while still being unsettling and dissonant.
** "A Smile Beneath Your Skin" is SurpriseCreepy in song form. It starts off as a pleasant chiptune theme, until it gets increasingly more and more industrial. Naturally, it was used in the [[https://www.youtube.com/watch?v=XGhqaCHt2Jw 2015 trailer]] for the game.
** "Breakfast Was Too Early" sounds somewhat nicer compared to the rest of the soundtrack... but there's something wrong about this track being played while you're being chased by Specimen 4.
*** "Breakfast Was Too Late" is an edited version of the track that sounds like it has been quite distorted.
*** And then there's "Breakfast Was Too Water Damaged", which is the same track, but being played backwards.
* Think about this: The CAT-DOS entries for the last 3 Specimens (Specimen 9, New Specimen 10, and Specimen 11) share one common theme, which is that their containment procedures are (or were, in Specimen 9's case) failing. Specimen 9's entry claims that Specimen 9 was destroyed, which clearly isn't the case given the endless hallway, Specimen 10's entry implies that Specimen 10 was on schedule to be destroyed (given the "deemed too hard to contain") but escaped before they ever got around to killing it, and finally, Specimen 11's CAT-DOS entry outright says that no matter what they do, Specimen 11 can escape the House. And given that the same containment failure happened with Specimen 3, which is arguably the least threatening Specimen, what's to say that all of the Specimens aren't just completely loose in the house?
** Considering that pretty much every Specimen can chase the player at any time after initially encountering them, this seems to be the case. Meaning [[NiceJobBreakingItHero that it's because of the player that the are currently roaming free around the house]].
* Speaking of CAT-DOS entries, this is Specimen 11's effectiveness evaluation: "Proved not effective as victims souls do '''not''' remain after contact". Which means that the Game Over screen - "[[AndIMustScream Your soul wanders an endless maze]]" - it's '''NotHyperbole.'''
** Which brings up its own FridgeHorror. That either Specimen 11 traps your soul in its own pocket dimension or that it somehow [[CessationOfExistence destroys your soul completely]]. Although the latter could be considered a mercy.
*** It's the former - the game over screen still says that "Your soul wanders an endless maze". [[FridgeHorror It doesn't specify]] ''[[FridgeHorror which]]'' [[FridgeHorror maze]].
* While not NightmareFuel for normal people, for claustrophobics the air vent passage in Specimen 10's area and the play tunnels in Specimen 11's room can be terrifying.
** Even to those who are not claustrophobic, the build-up to Specimen 10 can be pretty terrifying. You enter a dark sci-fi-looking area that wouldn't look out of place in ''VideoGame/DeadSpace'' or ''Film/{{Alien}}''. This mitigates a bit when the lights are turned on. However, later in the area, the lights go out again, and the only way forward is a vent with dried blood on the floor. When you enter the vent, you may notice sounds while you're crawling through the vent and walls of goop that you have to cut through. And when you finally leave the vent and turn on the power switch, you meet with the above-mentioned Specimen.

* The fact that at least 4 people died from Specimen 1 attacks is seriously horrifying. Specimen 1 are nothing but cardboard cut-outs, after all.
** To be fair though, those that died likely had heart problems. Still, it's hard not to imagine the situation where a person narrowly escapes a Specimen just to fall victim to a simple jumpscare.
** Of course a bit of NightmareRetardant when you consider they might've died because Specimen 1 could've just whacked them really hard in the face. That makes it funnier, but still. That'd hurt.

* Karamari Hospital is out and proves to be full of MindScrew. Of notable mention are the lead-ups to the monsters. Probably the scariest thing about them is that unlike the monsters listed above, there are pretty much no documents which foreshadow them or explain what they are.
** The [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/a/a2/3DPolice.png/revision/latest?cb=20160102220929 first monster]] is what appears to be a security officer with [[BodyHorror hideously swollen hands]]. He is not terribly dangerous by himself, but he can give some self-made jumpscares when you have to backtrack through the hospital, likely forgetting him. Oh, and one of the doors near him contains a room that has a bed with blood rising from it where you can hear screaming in the background.
*** Another room can be found in this area as well, which contains just a single chair. If the player sits in the chair for a while, they may start to see an image of a [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/0/08/HOS_ex4.png/revision/latest?cb=20151230235517 disturbing face]] smiling at them. Eventually you are forced out of the chair, with a ghostly apparition leaning in it. On a NewGamePlus, killing it and sitting back in the chair causes a NonStandardGameOver where the player character wakes up in an alien desert, with the feeling that something is trying to take control of them.
** The [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/0/02/3DBodyBag.png/revision/latest?cb=20160102220949 second monster]] appears in the morgue, where, after the player gets an emblem for a puzzle, the bagged-up corpses suddenly disappear. When you exit the room, you are in a hallway with them lining the sides, leading to one that suddenly rises and chases you, which you then have to run from through a hallway maze where it can head you off. Oh, and when you get to the end, you notice the corpses again, except they are levitating.
** The [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/e/e1/3DBaby.png/revision/latest?cb=20160107172726 third monster]] is probably the most bizarre one. It can be found in the basement and is a shifting distorted baby-like face in a 3D cube that can move through walls. Also, it emits an eerie sound when it is nearby and overall has an UncannyValley feeling to it.
** The [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/9/98/3DHangman.png/revision/latest?cb=20160102221030 fourth monster]] is a disfigured child-like figure in hospital scrubs that spawns when you get the key in the basement. He has a bandaged-up face, but his open mouth looks a little demonic. This chase sequence is more difficult because not only is the basement much more of a segmented area, but the boy can also teleport, even if you are looking at him.
** Finally, there's the [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/2/20/3DGhostCow.png/revision/latest?cb=20160102221043 fifth monster]], encountered in the isolation room: a rotting upside-down cow with red eyes and a SlasherSmile. Might sound silly, but the situation in which you face it is tense. You are in a wire-fence maze where suddenly this thing attacks you from behind. It is ridiculously fast, able to catch you while sprinting and the maze has several dead ends, which pretty much guarantees you getting hit by it. And if you manage to be able to look at the top of its model, you can notice some sort of mouth-eyeball thing controlling it like a puppet.
** Probably the creepiest thing about this hospital is that it is so unknown.
** There's also the fact that ''Karamari Hospital'' averts the [[StrictlyFormula standard formula]] of the main game: reach a checkpoint, go through 10-20 rooms, enter new Specimen room, eventually encounter Specimen, run like hell, then beware of random encounters until the next checkpoint. ''Karamari'' has no "generic" rooms nor obvious Specimen rooms, which means on your first playthrough you never know when and where something will attack you.
** The Endless Mode Update also added some new things to the hospital - first one is a seemingly fake door leading to the Deformity Wing, which takes the EldritchLocation aspect of both the house and the hospital UpToEleven (highlights include entering a room and exiting through the same door, which leads somewhere else entirely), the whole wing dragging on endlessly, and when you finally do attempt to return, suddenly you have to get through a much bigger stretch than when you made your way in. [[MindScrew It's kind of hard to explain]].
*** The second one is a seemingly innocent minigame - you find a small Gameboy-like device in the basement, which seems to have a DatingSim game of sorts.

* Karamari Hospital is out and proves to be full of MindScrew. Of notable mention are the lead-ups to the monsters. Probably the scariest thing about them is that unlike the monsters listed above, there are pretty much no documents which foreshadow them or explain what they are.
**
The [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/a/a2/3DPolice.png/revision/latest?cb=20160102220929 first monster]] is what appears to be a security officer with [[BodyHorror hideously swollen hands]]. He is not terribly dangerous by himself, but he can give some self-made jumpscares when you have to backtrack through the hospital, likely forgetting him. Oh, and one of the doors near him contains a room that has a bed with blood rising from it where you can hear screaming in the background.
*** ** Another room can be found in this area as well, which contains just a single chair. ** The first unique monster you encounter, dubbed Woormy Charles, initially appears to just be a misshapen doll, but quickly stretches out to reveal that it is some kind of [[HumanoidAbomination bipedal tentacle monster]] wearing a baby doll's head. Unlike most other monsters in ''Spooky's'', it can ''sprint'' after you, and will do so at every opportunity. Even if you duck into another room to escape it, you can still hear it wandering around outside making some unsettlingly squishy noises. The way it kills its victims is by ''stretching'' them until their skin splits, which is lovingly detailed in its [[HaveANiceDeath death screen]].
** In the Great Hall, there is a brightly colored room with a MonsterClown standing in a cage. While the clown will be completely motionless as long as you keep it in your field of vision, looking away for even a split-second will cause the clown to suddenly be in a different position when you look back. ** At a certain point in the DLC the floor will collapse beneath the player, causing them to fall into the Root Cellar. In order to escape, they must fumble around in almost complete darkness to find the missing pieces of a door mechanism. One of these pieces is found directly underneath a doll resembling a bloodied woman affixed to the wall by several hooks, who soon vanishes. In order to get the GoldenEnding, the player must revisit this area to retrieve three black candles, only now the doll will actively stalk them. Her gimmick is a combination of InterfaceScrew and DontLookAtMe, requiring the player to keep their heads down to the ground the moment they see static lest they get her anger and be strung up on the walls themselves. The note that clues you in to this gimmick is found by [[EyeScream a pair of eyeballs in a large pool of blood]], with the note itself heavily implying that this was self-inflicted by the writer.
** Be careful not to use the doll to heal too often, lest you let its mood drop too low(indicated by the doll covering her eyes). If you do, you run the risk of encountering [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/0/07/Webp.net-gifmaker.gif/revision/latest?cb=20201003235800 Frenzy]], who will appear in certain rooms and charge at you with a hideous shriek. Even if you do keep the doll's mood up, there's still a chance that Frenzy will appear if you aren't actively carrying the doll with you, and there are several segments that require you to be separated from the doll for extensive periods of time.

** One of the new specimens includes White Face from ''VideoGame/{{Imscared}}'', who follows you by floating in the air and seemingly teleporting around. The death screen certainly doesn't help, with it appearing like it doesn't get one, until it suddenly changes to White Face slowly [[TheFourthWallWillNotProtectYou approaching the screen]] and threatening you, while a corpse hangs on a ceiling with your name on it. Then a dialog box opens and as soon as you press close, the game abruptly closes. Thankfully, the game doesn't put any new files on your computer, but it is still rather shocking.
*** For added effect, [[ShmuckBait try typing "open" in the dialog box that appears after you get killed by White Face]].
** Another one is a wolf-like human girl named Tirsiak, and is actually cute-looking rather than scary. Her death screen, on the other hand, has her teleport you to a forest, where yellow eyes light up. Wolves start to rush the screen, slowly turning it red as you are EatenAlive. The game then shows you your body and... [[http://vignette1.wikia.nocookie.net/spookys-house-of-jump-scares/images/6/6d/MS24_06_spr.png/revision/latest?cb=20160706180534 it's not pretty]].

The DissonantSerenity of her voice doesn't help the slightest.
** Even though Otto - an animatronic otter that moves very slowly and deals little to no damage to you - was intended as a joke specimen, the way it moves still falls into the UncannyValley.
** Then there is Spooper, who, [[FluffyTheTerrible despite the silly name]], [[BewareTheSillyOnes manages to be genuinely terrifying]]. At first, it [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/a/a7/Spooper.png/revision/latest?cb=20160706174230 appears as some sort of a kid]] in a BedsheetGhost costume, blocking the door leading to the next room. And after you maul it one more time, [[SarcasmMode it gets even better]]! It will remain unseen most of the time, but you will still have your health being drained constantly while it chases you, so you cannot afford to linger in any room for too long. Oh, and sometimes, it may root you in place and make you vomit blood, so that you begin losing even more health. After vomiting, sometimes there's a fetus in the blood.
** The most horrifying one out of the new specimens, however, has to be Lisa. From the build-up (the screen turning redder with each room and notes appearing that seem to be [[TheFourthWallWillNotProtectYou directly addressing the player]]), to her overall design (a half-dead humanoid whose head and neck constantly contort in all directions while the rest of her body stays completely limp), to the chase (basically, Specimen 6 on steroids, and she literally forces you into an endless loop of the same rooms), to the [[http://vignette2.wikia.nocookie.net/spookys-house-of-jump-scares/images/0/0b/MS25_01_spr.png/revision/latest/scale-to-width-down/552?cb=20160715032045 death screen]] [[NauseaFuel that will probably make you vomit your guts out]].

Oh, and her [[http://vignette2.wikia.nocookie.net/spookys-house-of-jump-scares/images/b/b6/RINGU_AMB.ogg/revision/latest?cb=20160706184035 new chase theme]]... [[HellIsThatNoise Yeah]]...
** The monsters from Karamari Hospital also make a return, and all of them are equally as horrifying as all the other monsters. Where do we begin?
*** Baby Face can appear literally anywhere when you are being chased by it. It's still very slow, sure, but there are still chances of you getting trapped in a thin corridor while this thing is blocking the exit.
*** There are plenty of reasons already mentioned above as to why The Hangman is horrifying, but it gets even better because it now has a [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/3/30/M4death.gif/revision/latest?cb=20160706165212 death screen]] (seizure warning)!
*** The Ghost Cow seems to be a RealityWarper of sorts, since there is no other explanation as to why it turns all rooms it chases you through into the same chain-link maze from the Isolation Zone of Karamari Hospital, as well as the fact that it [[TeleportSpam really likes to abuse the new teleport ability it received]]. Oh, and there is plenty of InterfaceScrew, too, as well as this [[SarcasmMode lovely]] [[https://vignette.wikia.nocookie.net/spookys-house-of-jump-scares/images/f/ff/CowDeath.png/revision/latest?cb=20160707215752 death screen]] it gets.
*** Bekka is no longer the NightmareRetardant she used to be -- she also has her fair share of InterfaceScrew, as well as [[LightningBruiser moving really fast and mauling you in less than a second]].

* Anyone who finds dolls creepy will certainly not be comfortable in The Dollhouse DLC, which features the player coming across a Victorian house setting that is quickly explained to be a GL Lab facility where they capture souls into dolls and train them, as shown with the doll you have to carry around to either heal yourself or open doors. Unfortunately, the soul binding process had not been perfected, with the [[CameBackWrong failed attempts]] littered across the house. The resulting abominations, referred to as Husks, resemble little bloodied dolls of children that seem to crumple into bags of bloody meat when killed.

The note that clues you in to this gimmick is found by [[EyeScream a pair of eyeballs in a large pool of blood]], with the note itself heavily implying that this was self-inflicted by the writer.

The resulting abominations, referred to as Husks, resemble little bloodied dolls of children that seem to crumple into bags of bloody meat when killed.
A note on a nearby desk explains that the clown was originally summoned to cheer up the children's souls, but [[HumanoidAbomination whatever it was]] instead caused the souls to be greatly distressed instead[[labelnote:*]]The Clown Room is one of only two locations where the doll will cover her eyes regardless of her mood, with the other being the graves of Spooky's parents[[/labelnote]], leading it to be locked away as a safety precaution. After a certain point, [[ParanoiaFuel the cage will be empty, and the clown will never show up again]]...if your playing the original version. In the HD version, the clown will instead randomly appear in certain rooms and slowly come closer while your back is turned, [[NothingIsScarier yet will never do anything beyond that]].
** Then there is the final room, which upon entering will trigger Spooky to appear and [[OOCIsSeriousBusiness uncharacteristically plead with the player to leave and that there is nothing for them here]]. In the center of the room there is a receptacle that tells the player that they must sacrifice the doll they have been carrying in order to end the nightmare. Upon doing so, the the player is transported to a new area resembling a cabin with empty bottles littering the floor, and must pick up a gun and fight their way through demonic apparitions that vaguely resemble [=WW2=]-era German soldiers. Eventually they reach the last corner and automatically fire a shot at what they percieve to be a devil. Unfortunately that "devil" is quickly shown to be [[WhamShot Spooky herself back when she was alive]]. Not only does this mean that you're forced to experience her final dying moments firsthand, but the note on the corpse that appears immediately after you return to the present strongly implies that the man who shot her during a PTSD-induced psychotic episode [[AdultFear was none other than her own father]].
** Finally there is the BadEnding that if you escape without closing the HellGate. While the spirits of the house are released, with nothing stopping the HellGate it starts churning out horrors that overtake the world and quickly turn it into a barren wasteland.
