* In ''ComicBook/ArkhamAsylumASeriousHouseOnSeriousEarth'', the scene where Batman stabs his own hand with a shard of glass had the script call for Batman to draw a small amount of blood to snap himself out of his daze. Dave McKean however, drew the scene with Batman driving the shard straight through his hand. Grant Morrison was delighted by this (since it made an already freaky scene absolutely nightmarish), and kept it in.

* One of Jack Benny's running jokes in ''Radio/TheJackBennyProgram'' was that he drove an ancient Maxwell automobile that was so old it barely ran at all. The sound effects for the alleged car were originally recorded from an actual broken down car engine, but one broadcast when the sound guy missed the cue, Mel Blanc grabbed the microphone and improvised the sputtering car vocally. It went over so well Jack decided that Mel would do the sound of the Maxwell from then on.

* Creator/JeffFoxworthy's debut album ''You Might Be a Redneck If...'' contains an example. Although he had opened with a few of his trademark "you might be a redneck" one-liners, an audience member can be heard shouting "Redneck!" near the end of the set. Jeff clearly stumbles for a few seconds before closing the set with more "you might be a redneck" one-liners. This may also explain why most of his subsequent albums feature the "you might be a redneck" jokes solely at the end.

** InUniverse example in ''Literature/TheSilmarilion''. Aulë, one of the Valar sent to craft the world before the coming of the mortal races, created the Dwarfs in secret so he'd have students to teach but could not give them souls. The god of the setting, Eru Ilúvatar, adopted them and gave them souls himself even though they weren't originally part of his plan for the world. This caused some strife down the line, but since [[SatanicArchetype Melkor]] had already screwed things up beyond recognition and they were specifically built to resist his corruption it worked out okay. The Ents also weren't in the original plan, but were made at the request of Aulë's wife Yavanna as guardians of the wild places.
