The second and third forms hitting HumanoidAlien territory only raise more questions. If one interprets Lavos to be more intelligent than meets the eye, it may also be duplicitous in manipulating Queen Zeal, seeing as she orders it to attack the party and it grants her the immortality she wishes in exchange for her handling the dirty work.
