I was looking up Kaguya-Sama's character pages as a reference for naming conventions, namely how Tropers refer to most characters by their last names in the entries. I also checked the discussion page and found that they do have a style guide for the pages; Characters are referred to their last name based on what the series’ Narrator calls them and/or what they’re called the most in the series.
Miyuki is Shirogane, Yu is Ishigami, etc. And Shirogane is referred to as “Miyuki” on his relatives’ pages to avoid confusion.
This was something I thought about considering most characters in both Kaguya-Sama, Komi Can’t Communicate and The Dangers In My Heart are referred to by last name. In fact, I usually see fans use “Ichikawa” and “Yamada”.
