I'm not really sure that pre-CGI live-action glow special effects should be counted as "underlighting."

My understanding was that underlighting was done directly on the animation table. In live action, the artist had to create a matte frame of film with a clear hole in it and then project light through the matte frame onto the final frame. The matte frame was suspended in a film projector, not laid on a table.
