Crazy Sane is a sane person who has to adopt a crazy personality to survive an insane world. It's effectively a Beneath the Mask sub-trope, where the mask is insanity and the hidden reality is sanity.
Only Sane by Comparison is a crazy person who, at first glance, looks like they're the Only Sane Man in a group of crazy people, until you realise they're genuinely crazy — they're just not as crazy as the rest of their group. It's effectively a subversion of the Only Sane Man trope — they appear to be the Only Sane Man, but it turns out they're not (they're just relatively less crazy than the group they're hanging around with).
If my post doesn't mention a giant flying sperm whale with oversized teeth and lionfish fins for flippers, it just isn't worth reading.
What's the difference between this and Crazy Sane?
