She uses a 3D avatar so she can be dressed however she wants and nobody will see. She did make it pretty clear that she gets high to stream, she just has to be sober for the rig setup.

So it's possible. "dressed however she wants" could mean "undressed". I mean, I game naked sometimes and I'm not a streamer.
