God Hand doesn't fall into that though, it establishes your playthrough of the game will be painful.

You got some dirt on you. Here's some more!Unless the game was made hard due to some crazy "HOW COULD YOU HAVE LEFT THAT IN?!" bug, I can't see how often it would be justified.
The few I can list off would be...Duriel's ability to kill you BEFORE you can finish loading(not a problem for warriors, but for mages), and ummm...that's about all I can think up atm...
There are all those really old games with really shitty controls, but that's not the game being hard, but the designer screwing up.

Anyway, hack and slashes...I never actually got what that meant. I mean, Diablo is sometimes named hack and slash or an action-RPG. However, Kingdom Hearts is also named an action-RPG. So is Phantasy Star Portable. Does hack and slash mean "killing higher and higher levelled enemies in order to obtain stronger and stronger gear" (Diablo, PSP), or does it mean "slashing through the opposition as a main way of playing {Nanobreak, DMC)?
The Latter, usually. DMC actually Helped make it popular today.

Back on Topic, I like Hack And Slash games a lot, and IGN lambasting them is pretty strange. They seem to have it out for Beat-em ups in general. Remember their infamous God Hand review?
