OK, heads up: I play video games only very, very little, so I'm not exposed to tropes related to video games much.
But this one could definitely use a new name and the description should be expanded — it mentions it looks like Cosmetic Award at first, so the comparison of the two could be elaborated on. Does it subvert the Cosmetic Award perhaps?
Also, it needs a bit of clean-up with Example Indentation and general example misuse like this one: "Many MMORP Gs nowadays attach small stat bonuses to their Titles. Developers occasionally get carried away." It's my understanding that broad generalizations about recent development are not allowed to be posted.

I don't see how personally. Video games with an Achievement System basically offer nothing but cosmetic awards 99% of the time. It's the 1% that don't that comprise What Do You Mean, It's Not Cosmetic?. Hence the name I suggested.

I'm not really sure we need a separate trope for what is essentially a simple subversion of Cosmetic Award.
But anyway, since the trope references Cosmetic Award, that is what the name should reference too. Not Achievement System.
Also, not all bonuses given by achievement lists are necessarily examples. Achievements only "look like a Cosmetic Award" in games where most of them actually are.

What Do You Mean, It's Not Cosmetic? is an somewhat old and well-crosswicked page, but it has a line of dialogue title and only 5 inbounds - a common consequence of line of dialogue titles, especially when they are also snowclones. Which is a bit of a disappointing performance.
