Hey, we didn't say every pilot was smart.


  • Two passengers of Asiana Airlines Flight 214, who both died due to not wearing their seatbelts despite the light being on (the third death was bad luck).
  • The pilots of the 2008 Mexico City Learjet crash, who both faked their credentials.
    • Speaking of which, it's likely nobody checked the credentials before the flight. Even a small check would've shown inconsistencies.
  • The person who used the wrong bolts on British Airways Flight 5390 (he checked by using his regular eyes, instead of checking what bolts he grabbed).
  • The person who placed Oxygen Generators on ValuJet Flight 592.
    • Not only that, baggage handlers also confused them with Oxygen Canisters, and confused "Expired" with "Empty". Had the flight's pilots knew what was actually onboard, they certainly would've told them to unload them.
    • Not only that, the handling of the packaging was also poor. One even loosely wrapped them in freaking bubble wrap.
  • McDonnell Douglas themselves. After two cargo-door blowouts, people were given access to the DC-10 testing archives. It was found out that one test had its cargo-door blowout, but the design remained fundamentally the same.
    • Also, after the first blowout the recommendations were already made. However McDonnell and the FAA (Federal Aviation Administration) both agreed it was too expensive, and improvised.
    • It wasn't until after a lawsuit that McDonnell changed the cargo-door design.
  • American Airlines, United Airlines and Continental Airlines, who all used a faster way of removing the engine for repairs (normal procedure would be detaching the engine, but the faster method was to detach the pylon, then reattach it with a forklift). After American Airlines Flight 191, multiple other DC-10s were found to have the same damage, and the faster method was banned.
  • Speaking of American Airlines, they used to teach pilots to use the rudder vigorously. On American Airlines Flight 587, the vigorous movements broke the rudder off the A300. After the accident, American Airlines modified their training.
  • The FAA allowed highly flammable material on aircraft. After SwissAir Flight 111, the material was finally banned.
  • Pan American, who was violating standard procedures on loading cargo (they were found to have not checked if packages had their respective owners on the flight). 3 years after the bombing of Flight 103, Pan American filed for bankruptcy.
  • Scandinavian Airlines for not telling their pilots of a new safety feature (that was designed to increase engine power at low-altitudes). It was a miracle that everyone on Flight 751 survived.
  • The hijackers of American Airlines Flight 11 and United Airlines Flight 93, who both accidentally sent transmissions to ATC instead of passengers.
  • Continental Airlines, which authorised installing untested and unsafe spare components to their aircraft. This culminated in a strip of metal falling off a DC-10 at Charles De Gaulle Airport, which was ran over by the Concorde operating Air France Flight 4590 on take-off, rupturing a tire and causing one of its massive Olympus engines to explode, causing the plane to lose altitude and crash into an airport hotel.
  • Air Florida Flight 90's pilots, who didn't deice before take-off.
  • The pilots of Delta Air Lines Flight 1141 who were distracted by casual chitchat with the flight attendants forgot to extend the plane's flaps and slats, resulting in a crash shortly after takeoff.
