Takeru resolves to never hesitate again and to open his heart to the heroes’ hearts, as his Dad once told him to. The Sage gives Takeru his next hint, “The strongest gunman in the West! He died the way he lived.” As usual, Takeru gets the hint right away: Billy the Kid.
A man comes to the Supernatural Phenomena Research Lab to make a request. His best friend has had a sudden change in his personality, almost as if he was an entirely different person. Shibuya and Narita also come bearing news of sightings of the Mysterious Man, Saionji.
Takeru decides that they should handle both cases, with Takeru investigating the best friend while Akari and Onari pursue Saionji. Meanwhile, Spectre has his own encounter with Saionji, whom he recognizes from an incident that took place 10-years-ago… Later, as Akari and Onari track down Sainoji, they witness him begin his preparations to produce the Billy the Kid Ghost.
As Takeru investigates the best friend, he finds that everyone in the same apartment building has changed. A Ganma appears before him and he chases after it as Ghost. Spectre appears once again. This time, however, Takeru discards his fear and confronts Spectre!
- Dragged Off to Hell: Revealed to be why Makato and his sister vanished ten years ago: they were sucked into the Ganma dimension by one of Saionji and Takeru's father's experiments.
- Product Placement: A mild example: The Kamen Rider Drive and Ghost magnets pre-order gifts what come with the tickets for the Drive X Ghost movie appear in the beginning of the episode.
- Wham Episode: The Mysterious Man's identity is revealed, as is Makoto's relationship with Akari and Takeru, and that he and his sister (who's in his Eyecon) were trapped in the Ganma world. However the biggest wham is that Saionji and Takeru's father are old partners. Not to mention how this is the first episode to end on a cliffhanger, akin to the Monster of the Fortnights of other Riders.
