Sherlock and Watson investigate a death in the hospital.

Tropes:

  • Addled Addict: One suspect is Dr. Cahill who's visibly twitchy while being interrogated. Gregson thinks his behaviour is a sign he's the Angel of Death but Sherlock notices that the man actually calmed down when they mentioned the Angel. It turns out Cahill's twitching is because he's been using morphine to get through shifts and believed they were questioning him about his stealing the drug from patients in the hospital.
  • Doctor Jerk: Mason Baldwin openly admits that he doesn't care about whether his patients are suffering, claiming that it actually makes him a better surgeon if he doesn't connect to them. This is somewhat undermined when it turns out he has two strikes against him for mistakes during surgery, and that one of the "Angel" victims was actually him trying to cover up a third mistake.
  • Even Evil Has Standards: Danilo Gura insists that he only killed people who were already terminal and gets angry when Sherlock points out that one of his victims was actually recovering. When Sherlock works out that he was manipulated into killing someone who was recovering, Gura willingly gives up the photographs he took of the forged medical chart so the doctor that altered them can be brought to justice.
  • Lab Coat Of Science And Medicine: The barista assumes the woman Holmes and Watson are looking for was a doctor because she was wearing a labcoat when she bought her coffees. The duo immediately note that no responsible doctor would wear their labcoat outside of work and risk contaminating it. The "labcoat" is actually part of the uniform at a high-end beauty store that wants to give the impression of being "scientific".
  • Mercy Kill: Danilo Gura sees his actions this way. The people he kills are all terminal patients and their deaths were a "release" from their suffering.
  • Serial Killer: Danilo Gura is the Angel of Death subtype: his victims are all terminally ill patients at the hospital.
  • Well-Intentioned Extremist: Gura genuinely believed he was helping people who were suffering by ending their lives.
  • Worthless Foreign Degree: Gura has a medical degree from one of the top medical schools in Ukraine and speaks near-perfect English, and yet in the USA he's working as a hospital janitor and not a doctor. The implication is that he was trying to fly under the radar as Ukrainian officials were catching on to his Angel Of Death activities.
