Plankton and SpongeBob need Karen to operate the time machine they had built, and remove her monitor from her robotic body. SpongeBob acts like he's Squicked out about it, saying "I've never carried a head before". Plankton responds "Don't worry, you'll get used to it," implying he's cut off and carried actual heads around before. Since Karen is a computer, this doesn't harm her, but she does state that powering the time machine is going to take up all her processing power.
