Kaguya didn't take seriously Ishigami encountering the six first mysteries as it was a side effect of her quest, but a few weeks later, Ishigami was very close to falling to his death (seventh mystery). Iino saved him, but without her, it's not that hard to imagine Ishigami falling to his death and Tsubame hanging herself in guilt.
