The game follows the titular Mad Sisters, a duo of supervillains consisting of Marisha (the human) and Yelena (the ball), joined by their ally Sashka, a skeleton that looks like a gun. as they wreak havoc on their unnamed city on [[ExactlyWhatItSaysOnTheTin (when else?)]] their week off. Along the way, they cross paths with Wrangler, an anglerfish-themed superhero who unintentionally leads them to steal the Shufflifier, a piece of technology that can rearrange a person's pixels, allowing them to turn into anyone. You play as Marisha, as you use Sashka as a gun to shoot at Yelena, who bounces around knocking civilians into the air.
