The reason why it primarily manifested the heroes' fears is that human minds, while incredibly creative, lack the knowledge and control to harness the power of creation beyond basic impulses, like projecting their own personal horrors.
** Sarge and Izel come from a realm where the inhabitants have no physical form, but long to experience one. When Coulson went in to seal off the dimensional rift last season, the act of conquering his fear subconsciously unlocked the potential to wield the Creation monolith, which sent a copy of his body back to the spirits' realm in the distant past, becoming an unwilling host to one of its inhabitants. With an echo of Coulson's memories confusing him, Sarge's only drive was to hunt down Izel, another like him.
