Even if Shinobu did pick up on Hayato's distress and try to do something about it, what happened to him would be so surreal from an outside perspective that it'd probably be dismissed as a bad nightmare or an over-active imagination rather than something that actually happened. This lead people to theorize early on that [[StableTimeLoop Josuke somehow ended up in the past and saved himself]], but the similarities in appearance can be explained much more easily. Josuke barely remembers the man who saved him beyond the pomp. Not to mention it's being told by Koichi, not Josuke himself. Josuke's Savior is simply represented in Koichi's retelling of the story as a man who looks very similar to Josuke, because Josuke's the closest anybody has to his appearance.
