Taking into account the distance between each floor, even if he only set the elevator to go up one level it's very likely that the noose was pulled much tighter than was necessary to strangle him or break his neck. There's a very real possibly that his hanging turned into a decapitation.

She tells you she felt so light that she thought she could float down safely, and then you receive a vision of the fall. It's bad enough that this is likely exactly what Amy saw, but two details drive it home even harder: First is the high pitched scream you hear near the end, partially hidden under the violin sting. Second is the way the camera slowly moves to the side as it falls, until it sits directly over a banister at the bottom. Amy didn't land on the stairs proper. She broke her back or neck bouncing off the banister first.

* Harvey's death is arguably the least horrific of the four we're shown, if only because he was stabbed and went down spouting a fountain of blood. Except his movements before going down indicate that he either attempted to pull the knife out himself, or grabbed at Jimmy's weapon arm in an attempt to stop him. The sounds he makes imply that knife went all the way into his esophagus; there wouldn't be that much wet gurgling otherwise. Harvey was either throat cut or partially decapitated by a sharpened letter opener, and took long enough to bleed out that he was still able to clutch at Jimmy's arm and loudly attempt to call for help all the way until he hit the floor - if not longer.
