** In one challenge of Survivor: One World, players have to memorizes a series of objects, then recreate the order at their own stations. In addition, there is a lever you can pull to cover the objects in hopes of causing your opponent to stumble. When it's Troyzan and Kat's turn, the former hits the lever before either has a chance to get a good look at the objects, with Troyzan's plan being to just put random objects down and hope he's right. After naturally getting it wrong, he continues to do this until he's forced himself and Kat to repeat the challenge '''seven times'''. And to make it even more of an epic fail, Kat ends up scoring her team a point, meaning that Troyzan's plan completely backfired on him.

* Frank Spencer from ''Series/SomeMothersDoAveEm'' pretty much embodied this trope. He tried to fix a toilet in a hightech house (which he'd broken when he found using it to be trickier than one might expect) and ended up stuffing it full of various household goods (including, if memory serves, a plant), flooding the house's electrics, causing every piece of equipment to go crazy and basically resulting in it nearly killing the occupants.

* ''Series/OddSquad'':
** In "Soundcheck Part Deux", the way Danny T's bandmates got separated from him was by having all of them be blown by a fan out of the window. While the impossibility of this is already justified due to the world of the show, the epic fail here is that ''the fan isn't even turned on'' when it blows them away -- never mind the fact that there doesn't appear to be a window to begin with.
** When disguised as the villainess Kooky Clown in "Undercover Olive", Olive goes through a myriad of these. Her EvilLaugh is passable at best, she's a BadLiar who, by sheer luck, manages to make most of the villains believe she's a real villain up until the climax, and she tries to convince Odd Todd that she's really Kooky Clown even after he rips off her clown afro wig and exposes her to the rest of the villains (and fails miserably). However, her biggest fail by far is her attempt to make a balloon animal to cheer Fladam up after he becomes enraged enough to attack her. She feebly blows into the balloon, hands it to Fladam, and tries to pass it off as a sleeping snake before making a quick escape.
** It's revealed in "O vs. the Ballcano" that Oprah has lost numerous Headquarters throughout her career as an Odd Squad Director -- one every year from 1984 up until the year before the episode takes place. Had Olive and Otto not intervened and helped during the episode, she would have lost another one. Considering that Odd Squad Headquarters are large establishments with numerous amenities, and considering that Oprah is usually a {{Determinator}}, it's quite astonishing how she is still even employed with the organization.
** In "Odd in 60 Seconds", both Lady Bread and Wheelie Dan attempt to reach the vault where the Odd Squad mega-computer is stored, which is behind three doors, in the span of a minute. Lady Bread is unable to get past even the first door due to her hands being made out of loaves of bread. Wheelie Dan, on the other hand, does make it through the first door, but fails to even make it halfway to the second door due to marbles being spilled on the floor as part of a trap, sending him flailing about. It gets to such a point where the agents end up feeling ''pity'' for him and ask him if they want to stop the timer, which they are using to time the suspects.

The first cook of said team, the captain, planned on making a mussel coconut broth with tomato, saffron and basil, accessible yet flavorful seafood dish so that other team members could continue without making much of miscommunication. However, the third team member "went rogue," deciding to change the menu entirely and overcomplicate it by adding around ten more ingredients including a ''white chocolate'' to make a velouté with. The next cook after him was overwhelmed by the amount of unfinished element on the station and the last cook ended up making a simple prawn with scallop ceviche dish to salvage everything in 15 minutes, sending them to elimination challenge. The incident was then called the "white chocolate velouté incident." A quite [[https://www.pedestrian.tv/entertainment/masterchef-drama-john-and-his-white-chocolate-bullshit-dodges-elimination/ ranty review]] sums this up quite well.
