* In the Comicbook/JonahHex comics, Tallulah Black is the titular protagonist's DistaffCounterpart, sporting a heavily-scarred face and missing one of her eyes. In the live-action movie, she's played by Creator/MeganFox, has both eyes intact, and is missing her trademark scars.
* In the comics, Comicbook/DoctorDoom wears a mask to hide the severe burns he suffered as a young man. The first Tim Story Fantastic Four movie keeps the mask, but the explanation is changed from a disfiguring explosion to his new powers gradually turning his body into living metal. The second movie then completely undoes this by having the Comicbook/SilverSurfer accidentally heal his face with an energy blast, which somehow conveniently lets Doom keep his powers and his good looks. This also raises the question of why he even bothers to don the mask during the final act, since he no longer has any reason to cover his face.

* . In the book Dracula was old and ugly, with hairy palms, whilst most of the films make him younger, suave, and TallDarkAndHandsome. The most extreme example must be the 1979 version, which makes Dracula look like this and have a voice that could melt butter. Same often goes for his personality; in the book he was an old man, tired with immortality, who wanted to take over the world, whose bite seems to be a metaphor for rape and sexually transmitted disease. In the movies, he seduces Mina and/or Lucy, with plenty of angst over the loneliness of living forever and being a vampire. Luke Evans who plays him in Film/DraculaUntold is a similar example.
** Also applicable to Renfield in one known 1931}} adaptation of the novel. While he's normally interpreted by older-looking men, more similar in appearance to the character in the books; he was played by the handsome Dwight Frye in the 1931 version.
* In Creator/StephenKing's Literature/Carrie, the title character is overweight, has thinning dark hair, acne in her face and chest and back, and possibly hirsutism (she seems to have all the signs of Polycystic Ovarian Syndrome- hence explaining her late puberty). In the movie, she is played by Sissy Spacek, who is slender, has thick shiny blonde hair, and clear skin- and thus her late puberty is likely due to something else! Though the filmmakers did at least acknowledge this by having the gym teacher tell Carrie that she is a pretty girl but just needs to work on her appearance. ** Inverted with the character Helen Shyres. Although a background character in the book, she's nominated for Prom Queen - implying she's attractive. The 1976 film cast Edie [=McClurg=] who is chubby and the character is presented as a ButtMonkey for the popular girls - also not appearing to have a date at the prom. ** Eddie Kaspbrak in the book is described as "rat-faced", these words really that don't really apply to Adam Faraizl or Dennis Christopher, and especially not Jack Dylan Grazer Creator/JackDylanGrazer and James Ransone.Creator/JamesRansone.
** Simarly Richie Tozier Tozier, who had buck teeth in the book, in Miniseries he's is played by normal-toothed 90s chick magnet Seth Green Creator/SethGreen (miniseries) and Finn Wolfhard who are Creator/FinnWolfhard (films), both more attractive than the book's description. Zig-zagged with the adult portrayals portrayals, as while the Miniseries's miniseries' Harry Anderson Anderson’s Richie isn't bespectacled, it's the handsome Bill Hader's Hader's portrayal that makes book Richie look like Quasimodo in comparsion.
** Adult Mike Hanlon gets this a heavy dose of this in the second movie, in movie. In the book Mike Mike, thanks to staying in Derry Derry, has been aged beyond his years and had his health effected. In It Chapter Two Mike is played by Old Spices Isaiah Mustafa Mustafa, who is very fit and handsome.
** The titular monster also zig-zags this, Tim Curry this: Creator/TimCurry, thanks to make up up, accurately fits the desciption of the ugly MonsterClown, while Bill Skarsgård Skarsgård can't help but be handsome despite the heavy make up. However Skarsgård still manages to be far more horrifying and replusive than Curry's portrayal, so it's a borderline example.

* In the original Manga/Gunnm manga, Yugo/Hugo is a teen of 15 at most, kinda scrawny and with a boyish face. In the LiveActionAdaptation Film/AlitaBattleAngel, he's played by Keean Johnson, who's considerably more built and has a certain '90s heartthrob look to him, not too dissimilar from the likes of Jonathan Taylor Thomas. It's particularly evident given this version of Hugo gets a brief ShirtlessScene whereas the original didn't.

** Subverted with Beverly Marsh in the book Bev is descibed as a stunningly beautiful redhead. In the Minseries she's portrayed by the more homely and brunette Annette O'Toole and Emily Perkins. The film versions have Creator/SophiaLillis and Crrator/JessicaChastain who effectively match the book’s description (though Lillis goes for the UnkemptBeauty route).
