Literally? the Omnicidal Maniac and anyone accused of crimes against humanity. As well as Groups who would seek to killAllHumans for one reason or another (example: The Daleks)

The actual use? Well that's a tough one. But I think the concept might found in cases like Star Trek and Star Wars where Humanity is unified under a single banner (The Federation) and are opposed by an evil empire, genocidal villains or Space Pirates. But the actual use of the term seems to be blurry with some extentiosnt hat would make sense (terrorists and "unlawful enemy combatants", possibly referring to PMCs) wind up not being covered by this principle.

The Usual Adversaries would probably cover it (since the term covered criminals whom could be prosecuted by any country with no regard for jurisdiction, and were therefore considered everyone's enemy)

The Usual Adversaries doesn't seem that fitting, IMO. It sounds more like it's about 1) the adversaries in question being exceptionally persistent in being a frequently recurring antagonistic force, and 2) the characters who regularly face those adversaries being justifiably annoyed/frustrated by this persistence (typically manifesting in complaining about the issue, e.g. "Why is it always orcs?! They just keep coming and coming even when the Evil Overlord who created and led them is Deader than Dead!"). It even describes itself as the narrative counterpart to the gameplay trope Goddamn Bats.

What about 0% Approval Rating or Always Chaotic Evil?

They seem to at best overlap with the concept, depending on the context. 0% Approval Rating is for individuals (usually figures of authority) that are universally hated by their own subordinates, and occasionally even their own peers. Always Chaotic Evil, on the other hand, is about Card-Carrying Villain applied to a whole race; i.e. in RPG terms, each and every single member has the same Character Alignment — specificaly one of the Evil ones — and is open about that status (unless Villain With A Good Publicity-style deception is also one of their race's signature traits), barring the rare Defector from Decadence. Neither of the two necessarily entail the individual or race in question being designated as an enemy to all humankind / all sapient races.
