Meaningless Villain Victory

In the fantasy film Quest For The Golden Dove, things look pretty bleak in the climax. The villainous Lord Conqueror has given the protagonist a disturbingly brutal No-Holds-Barred Beatdown, and then he opens the chest containing the Golden Dove, which is supposedly a solid, 24 karat gold statue of a dove that belonged to an ancient kingdom. There is indeed a statue of a dove, but it's a wooden one that has been painted gold. As it turns out, the Dove was stolen by a thief centuries ago and replaced with a fake one. The protagonist had already figured this out by now, and despite his injuries, he can't help but laugh at Lord Conqueror, who proceeds to have a Villainous Breakdown.

In the tabletop wargame Tabletop Game/ Star-Knights of Invernesse, Named Characters have the "Look Out!" Special Rule, which forces a nearby Red Shirt to take fatal damage for them. Captain-General Duncan Hertz procs his Unstoppable Rage passive skill if this happens to a model in his retinue.

In the B-Movie Attack Of The Killer Lemurs, the lemurs lure Steve to his doom by stacking on top of each other to form the shape of a beautiful woman and standing in front of a bright light so that Steve only sees the silhouette.

In one episode of Mr. Cat, the eponymous cat was pushed into the ocean by his rodent archenemy, resulting in him popping up with a starfish on his back.

In Tabletop Game/ Star-Knights of Invernesse, the Deathlords of Ceres have the Gorecrow swarming unit, which is made up of 12 little black birds. Gorecrows inflict the "Thousand Cuts" status effect on any non-mechanical infantry unit they attack.

In Light Novel/ What is That; a Goblin? Wizard trips over his robe, falls down a flight of stairs into a lobber trap, and is bodily thrown into a spike wall, somehow missing each spine. He stands up and shouts "I'm OK!" whereupon a single pebble detaches from the ceiling and somehow does 9999 damage when it nails him in top of his head and kills him dead as a rock.

In the "Butcher of NeuYork" story arc of Comic Book/ The Red Spectre, Red Spectre breaks his vow of no-kill Pacifism when the Butcher sets fire to an orphanage. He eventually ends up killing Butcher in their final battle.

The original translation of the Sega Genesis-era JRPG Video Game/ Phantastic Swordsman has the mayor of the Hidden Elf Village greet your party with "Aw hell!" The PlayStation 4 remake shows that this was supposed to be a warm "Ah, hello!" People were disappointed because the original translation gave the impression that you'd interrupted her lunch break and she was annoyed, which was a lot funnier.

The "educational" kid's film Captain Healthy Food And The Vegetable Eaters is about a bunch of aliens that come to Earth to eat all its vegetables. Without vegetables, everyone starts eating junk food. The superhero Captain Healthy Food scolds the people of Earth for eating junk food, and then goes on a 30-minute lecture about why junk food is bad and vegetables are good, and then it just ends without resolving the story about the Vegetable Eaters in any way.

In the Literature/ Kingdom of Blood novel A Night of Knives, Baron Darkphyre lets slip that his father was killed with ground glass in his dinner. Inquisitor Dreadgrave never mentioned the murder weapon was glass powder, meaning that Darkphyre is up for the guillotine.
