I think there are two meanings lumped into one trope here.

  1. Focusing on a character's mouth to emphasise an important thing they are saying. Usually done for a single word. The character's whole face may be shown before and after. Example from the page: In nearly every Detective Conan episode, there is a point where we get a closeup of a characters mouth talking ominously, sometimes whey they are making their debut or saying something important.
  2. Cropping off a character's head or eyes to hide their identity. Usually done for a longer period. The character's whole face may be shown later, but is never shown before. Example: Dr. Space Claw from Starship is first seen through a video connection. He's slouching in a chair and only shown from the neck down, as is his wife, which hides that he is Junior's father and establishes him as "the most evil man in the universe".

The lack of a single clear meaning is also leading to it being used as a generic "mouth focus" page much like Foot Focus was once used for all shots of feet (example from the page: The 1985 film Weird Science: In a scene in which Anthony Michael Hall and Ilan Mitchell-Smith are having a conversation in the shower stall during a party, we get shots of their mouths.).

Proposition: Narrow Mouthscreen to meaning #1 only. Make meaning #2 into a separate trope called Crop Off The Head or Crop Off The Eyes (I can take it through YKTTW).

I remember seeing #2 somewhere else (while going through "tropes in Main/") but I don't remember what it's named.

I suggest soft split Mouth Screen instead.

[up][up] I tried going through the Eye Tropes index, but the closest I could find was Hidden Eyes, which isn't about camera cropping at all.

I support that proposal, different reasons for doing a thing make for different tropes.

I could get behind a soft split but I don't think it's enough because meaning #2 can involve showing much more than the mouth, so "mouthscreen" wouldn't be an appropriate term.
