Absolutely insane, even more so than you'd guess from the premise

In Fullmetal Alchemist, after Ed and Al try human transmutation, Ed goes into the gate of truth, and he goes into a tunnel where alchemical knowledge is forced into his brain. It's apparently very painful for him to absorb all the knowledge at once, and on top of that he loses two limbs in the gate of truth and only recovers one of them by the end. That is what reading this fan fiction felt like to me. One time, I saw a post where someone said they judge any given book's quality by how disoriented they are with their surroundings after they finish reading it. By that measure, this is the greatest short story ever written.

Anyway, jokes aside, I'd really recommend reading this if you're bored, because I promise the one thing this fan fiction is not is boring. Even if you don't know anything about tf2 or the short story this is based on. There are some very dumb ideas, and some things I personally really dislike (was Medic in this continuity a former Nazi?), as well as some things I enjoy (AM's "hate" speech from the original recreated by what is essentially G La DOS) and some things that are genuinely good (the plot twist at the end is great in a really knife-twisting way), but at the end of the day, it's less about the objective quality, and more about whether it is entertaining, and as I said earlier, it absolutely is.

The NSFW level is about the same as in the original story, maybe a little more. Contains Heavy/Medic, but if you're interested in tf2 shipping for almost any reason, you probably won't like this.
