Happened in the first season of The Tudors. There is one actor (his name escapes me) who always appears in the credits but rarely actually on-screen. Said actor had an accident and was unable to continue shooting past the first couple of episodes, so they more or less dropped the character but kept him in the credits.
I'm bad, and that's good. I will never be good, and that's not bad. There's no one I'd rather be than me.
