I had always assumed that he glued fake horns on. That didn't explain the ears, though. I'm looking forward to how this rolls out.

Go play Kentucky Route Zero. Now.The characters have all evolved a fair amount over the course of the comic without their personalities being radically altered. I think the same will happen with Lil' Evil—he'll come to terms with who he is and continue his Poke the Poodle ways, but it'll shake him up enough that he'll do it for different reasons than we might expect.
Maybe you'd be less disappointed if you stopped expecting things to be Carmen Sandiego movies.The end of this arc will be Lil' evil getting his memories back, and proceeding to be a jerk to someone, before giving him a pet the dog moment to show he remembers a little.

The comic is being a little OTT about it, but then again, it's like that with everything and it's all in fun mostly. In fact I can't think of any instance where something is treated seriously. Not to say that it doesn't have serious messages (usually the enviromental ones) but the humour never leaves and both the good and the bad are open to being the subject of a joke.
What really surprises me is that he didn't make L'il E's mother into a devil. What might his motivation have been for leaving her human?

The matriarchy was from back when the comic was still doing elseworlds/imaginary stories—think Calvin And Hobbes with more racy subject matter—and was depicted as an oppressive dictatorship that had banned stereotypically manly things.
The patriarchy is pretty clearly "real", whatever the hell that means in a 'verse with a Reality Zone, and is a Matrix-like force that prevents people from seeing the world is sexist against women.
Make of that what you will.
