This is purely audience reaction, although it can be teased by the creator - my personal favorite example of this is in The Adventures of Dr. McNinja, where the author loves to throw in snarky Alt Text notes along the lines of, "And then Doc dies. The end," knowing full well that his audience won't buy it.
"It's Occam's Shuriken! If the answer is elusive, never rule out ninjas!"
