  • That One Level: Due to the missions being randomized, some types are notorious for giving the player headaches.
    • Sentinel missions can become extremely frustrating, especially when Special Zombies appear and the entrance is a closed space, which will put you in a dilemma of either wasting Painkillers to slow time and have the chance to kill the SZ and keep the accompanying hordes at bay or keep shooting the entrance and risk the SZ entering and screw yourself out of a massive money bonus.
    • Seek & Destroy missions are obnoxious to play through because of their mechanics: you have to locate and destroy all loudspeakers scattered through the levels. However, it's easier said than done because the loudspeakers' noises attract Zombies, which means they will spawn more frequently than in the other missions. Therefore, you'll have to kill hordes after hordes with almost no room to breathe while locating the loudspeakers, having to rely entirely on Painkillers to stay alive (and pass through Zombie hordes without being attacked) and explosives to clear your way or find the hidden Golden Pigs.
  • Demonic Spiders: Apart from causing Red-Eyed Zombies to spawn more frequently and having lots of HP, Special Zombies themselves can also annoy the hell out of you with their powers, especially if you're playing in higher difficulties and certain types of missions, such as the Sentinel ones with the mounted machinegun.
    • Kamikazes may not be able to attack you directly, but they will blow themselves up whenever you're too close to them. Although killing it from a distance will cause it to blow itself up without killing you, it's easier said than done when you have to watch out for regular Zombies going at you from behind or are backed into a dead end with several spawn points.
    • Scienfists create radiation that can damage and kill you the closer you are to them. To make matters worse, the only way to quickly kill them is with melee weapons, which means you have to use Painkillers to increase your health and pray no regular Zombies are attacking you from all sides.
    • Vomitrons' spit attack is easy to dodge, but it will mess up your aim and cover your screen in red goo for a while if it hits you. Moreover, it will spit at you no matter how far you are and will always hit you if you don't dodge it in time. Furthermore, this and its high HP becomes a hassle in missions where you have to shoot objects.
    • Ragers will charge at you whenever at a close distance, killing other Zombies in their way and knocking you to the ground if the attack connects. Moreover, their other attacks are aggressive and they can take lots of punishment from ranged weapons when shot at the front. While it has a weak point at its back, shooting it relies on baiting the Rager into charging, then sidestepping out of the way before shooting at the weak point while it recovers
    • Panzers, while slow, are the most annoying ones. Not only do they have armor at the front that allows them to shrug off any attack you may throw at them and therefore require you to waste explosives to take it out faster, but their melee attack is a broad downward swipe that will knock you backward into the ground. Moreover, Panzers will also keep doing this without giving you time to recover, thereby subjecting you to a Cycle of Hurting along with nearby regular Zombies if you back yourself into a corner or dead end.
  • Goddamned Bats: The red-eyed zombies from the second game are faster, stronger, and more aggressive than their green-eyed counterparts, on top of being still intelligent enough to wield pipes as melee weapons. That said, they are a pain on the rear to fight, especially when the player is in a closed space where there are multiple openings for them to leap over or crawl under. This is especially true when a Special Zombie appears, since its presence causes even more zombies to spawn and you have to kill them or ignore them and run (and pray the Red-Eyed Zombies don't surround the S). Even worse, weapon-wielding ones cause even more damage while having a range advantage.
