Venus was stalked and killed.
The Cacophobia fight represents Venus being stalked and killed by a group of people for being "ugly", similar to how Isaac imagines his mother wants to kill him for being "sinful". Venus is abstracted as a lifeless mummy throughout. It also brings up the possibility that Isaac's troubles wouldn't stop at his mother if he continued to live.

Cacophobia references Isaac's gender dysmorphia.
It's been established within TBOI's canon that Isaac is someone who is explorative about their gender, with Magdalene being an alternate identity Isaac dresses up as sometimes while in public. With Venus being named after a female god of beauty and how it appears as an ugly mass of Gorn under its wrappings, perhaps Cacophobia as a role represents the views (and possible scorn) Isaac receives from being gender-curious, with their seemingly plain, outward appearance hiding an inner self rife with turmoil and self-loathing. To this end, Cacophobia's name could be in reference to how Isaac sees themselves as ugly and unnatural, and their desire to change their appearance/identity. Perhaps, then, the final scene represents Isaac coming to terms with themselves as a person, deciding once and for all what they identify as.

Tainted Golem Predictions
The only way to get rock trinkets may be to take a chunk off of T. Golem's body (sacrificing heart containers like a devil deal, possibly also giving broken hearts) with a new machine in his subway.
