Tom will change his name.
A recurring motif in Daemorphing is the names people choose for themselves — the Peace Yeerks all go by pseudonyms, Elgat Kar chose her surname when she reunited with her sister, et cetera. With all this talk about the Hork-Bajir being Tom's family now, he'll probably cast off his human identity and choose a Hork-Bajir name for himself at the end of the series.

Tobias/Toby-Sirinial-Sky will have the last POV
Poetry said in a comment that they've had the final scene in mind for years. Tob' was the first narrator in the series, so it would be a nice Bookend to have them be the last as well.
