  • In the penultimate level of Episode 3, set deep within the Fuhrerbunker, you can stumble upon a room full of dead Nazis and one surviving S.S. officer. After killing him, however, you soon find a bloodied portrait of Hitler with his head replaced with that of a demon. This is never explained in any way.
  • In Episode 4, A Dark Secret, Sleepy, the secret level is a cold, dark and frightening maze crawled with officers, if they are alerted to your presence, will hunt you down until there is nothing left of you. If you manage to get past them, and collect every chalice in the maze, a familiar demonic, maniacal laughter reveals that a secret has been opened elsewhere. In the secret, you keep progressing until you see the Shiny Gold Luger, and it's guards are screaming headless men who use grenades who will one-shot you if you fail to dispatch them.
  • The entire fight against the Ubermutant and the NME in R.N.D, the sixteenth floor in Spear of Destiny. The fight begins when you enter a tight room with Dr. Schabbs and Dr. Krist overlooking you and you flip a switch that unleashes the disgusting Ubermutant onto you, who in addition to both melee and ranged attacks with his cleavers and gatling gun, can occasionally throw all of his cleavers for an instant kill. After you blow off all four of the Ubermutant's arms off, Dr. Krist decides you are worthy of fighting his masterpiece, the NME, a lightning-fast and bad-tempered robot. It can kill even a fully equipped player in a literal second if they even let their guard down once. After you beat the NME in a hard-won fight, Dr. Krist leaves, satisfied with the data you have given him, and the Ubermutant suddenly rises up without warning, with the music provided the only warning before he turns the player into swiss cheese, and forces them to fight the NME all over again in an earlier save.
