• The Doctor dreams of himself as Doctor Who. On a meta-level, this is because the story is a TV Comic pastiche, and so referring to him as "Doctor Who" is in keeping with those stories. Why he'd imagine using the name in-universe, however, is open to speculation. (It may be linked to his fondness for using WHO on his car registration plates.)
