The goal is to guide a man named Brett and his pregnant wife Carley to a safe area by fighting through waves of Decayed and occasionally bosses. Some levels also have special objectives and side missions where players can earn upgrade points to mix things up a little.

!!Tropes found in this game include:
* ActionBomb: Bomber Decayed, which were equipped with explosive vests before they died.
* BloodlessCarnage: Decayed spurt a little bit of green gunk from their wounds, but they do not bleed.
* BoomHeadshot: Hitting the Decayed in the head does bonus damage.
* DestructibleProjectiles: There's a boss battle with a now-zombified friend of yours, Dylan. He was overwhelmed in spite of being entrenched at a gatling gun nest and possessing a rocket launcher - which he is still able to use against you. You must shoot the rockets, then him - and make sure to deal with the basic Decayed swarming in as well! Luckily, your car is somehow sturdy enough to survive multiple rocket hits, but even then, it's still a tough fight.
** The later boss, Bulk, is able to throw large stones that arc towards your car, but can be destroyed in mid-air with enough hits.
* EleventhHourSuperpower: Brett is given an RPG by a survivor doctor on the last level to help defeat the final siege of Decayed attacking Highland Park.
* {{Expy}}: The starting level of the game has obvious expies of famous retail chains in the background, like "Star Tea" and "Forever 40".
* GameplayGrading: Each level is rated out of three stars. One star is awarded for just completing it alive, but the others are trickier, like requiring Brett to hit 90%+ of his attacks or to avoid taking damage past a certain threshold.
* GatlingGood: Dylan, a boss zombie with a rocket launcher, also has a gatling gun turret. He'll try to use it sometimes, but it's fa
* MacheteMayhem: Brett has a machete for when the Decayed get too close. When upgraded, it cuts through all the Decayed right next to the car, even if they are on the far left or right from Brett.
* MolotovCocktail: One of the more powerful grenade items. Once its fully upgraded it excels at crowd control and can burn through entire hordes in seconds.
* MoneySpider: Slain zombies instantly drop gold coins.
* MookMaker: During the game's final encounter, [[FinalBoss Bulk]] can somehow instantly cause a doorway to pop out of the ground halfway through the field, which will spawn in more Decayed until it's destroyed.
* NotUsingTheZWord: Everyone calls the infected creatures Decayed.
* PregnantBadass: A few levels have Carley join her husband in fighting off a wave of the Decayed.
* RevolversAreJustBetter: A revolver is the highest tier of weapon available and has the best damage, even compared to the automatic weapons and shotguns.
* SawedOffShotgun: One of the unlockable weapons is a sawed-off shotgun.
* ShieldBearingMook: Later levels involve zombified riot police officers, who are almost entirely protected by their shields, and so can only be hit in the top of their head.
* ZombieApocalypse: The goal is to fight through waves of zombies and make it to a safe area before your wife gives birth to your new baby.
* ZombiePukeAttack: The FinalBoss, Bulk, can spit a huge wad of green goop that deals no damage, but fully covers the car and thus immobilizes Brett until it's cleaned off by tapping.
