Is it a hint that he's perhaps done some gender questioning and might want to use the name "Savannah" for himself? Is "Savannah" a name he'd intended to use for a character in his elaborate daydreams, but never got around to before the accident? Was some part of him still holding out hope that, somehow, he'd live to adulthood and maybe get to have a family, perhaps with a daughter named Savannah? (Especially in the version of the script where Ricky's muteness ''isn't'' due to a disease, but trauma; in that version, Ricky likely had every expectation he'd grow up, leave Uranium, and have a shot at building a life and family.)
