** When her (bank manager) dad is having her thrown out, one of the things he says to her is that her real dad didn’t live to see her birth. The implication I got from that was that he saw to it personally.

The blind woman that Manni borrows the phone card from seems to know when the homeless guy is about to ride by with the money. Seeing that she is blind, she would notice the repetition with sound and having heard Manni's issue from the previous loops, she realizes that he needed to get the money back to stop the loop. So, she tells him to wait so he could see the bike and get the money back.

The moment that Lola hangs up the phone is the beginning of the level. The live-action portions represent how the game characters perceive the game; the animated scenes represent how the players perceive the game.
* Specifically, it seems to be a Main/WideOpenSandbox with Main/RPGElements; the roulette wheel is probably some kind of Main/UnexpectedGameplayChange mini-game.
** And her scream is actually a LimitBreak.
* By "self-aware" do you mean "sentient" or "aware she is in a video game". Because she doesn't seem to know she's in a game.
** She's aware of previous iterations. In the first, when she helped Manni rob the convenience store, she didn't know how to use a handgun; Manni had to tell her how to turn off the safety. And the way Lola whispered "No" just before both of the reboots suggests that she has some control over them--or at least she thinks she does.

In between each of the three different scenarios it cuts to a conversation the two are having, about (for example) what if one or the other was about to die or similar. Could it be possible that none of the events actually happen but are just Lola (or perhaps Manni?) daydreaming of what might happen if he did get into trouble and thinking about the consequences?
