The episode opens up to the Griffins getting ready for Thanksgiving and they get a letter from Peter's sister, Karen, which states that, after spending Thanksgiving with their mother, Thelma, for 20 years up until her death, she wants to spend the holiday with Peter. Peter, being Peter, refuses to invite her but Lois insists.

When the day comes, Karen arrives, while the family is watching [=TV=], and everyone finds her delightful but Peter doesn't. When they do family football game, Karen performs the "Toxic Shock" on Peter, which chokes him out, and shocks everyone, especially Meg.
