* Shiki [[SanitySlippage losing his mind to Roa's influence]] is so much worse in the Remake.
** For a start, it is even more drawn out and agonizing for Shiki, to the point where there are gaps in his memories where the reader legitimately wonders if Shiki had become a serial killer off-screen. The extent of his madness is even shown more vividly with the damages he does to his own room and his increasingly chaotic point of view.
** Kohaku's role is incredibly unnerving, especially for those who knows her role in the original. This time around, Shiki (or Roa) notices that she is calm and unsurprised, like she had experience handling monsters in human flesh. And we know she does, because that is exactly what happened with SHIKI. Even as Shiki begins physically attacking her, her [[DissonantSerenity smile]] never waves, making one wonders how much active manipulation she took part in leading Shiki to this state.
** The worst part is, this is not even as bad as it can get. If Shiki [[JumpingOffTheSlipperySlope helps Noel "punish" vampires]], [[InLoveWithYourCarnage much to her delight]], it is implied that his more violent mental state makes him even more susceptible to Roa's influence, and his madness during the same scene is even worse, leading to him actively killing people, which he would otherwise have been able to hold himself back from. The damages done to his mind becomes even worse, and he begins to hear distorted voices from people who may or may not be there. And the worst part is he kills Akiha and Hisui [[ThroughTheEyesOfMadness without realizing it for two days]], with Kohaku covering it up and manipulating him into thinking everything is still fine, and him eventually killing her too. It culminates with Shiki coming to his senses, finding Kohaku [[HopeSpot alive]], before realizing that it is her reanimated lifeless corpse, to hammer home his vampirism. He runs to find Akiha, [[DeathSeeker believing she would be able to kill him]], only to find her and Hisui's corpse. Broken beyond repair, Shiki flees the now devoid of life Tohno mansion [[LaughingMad laughing]].

* If Shiki chooses to be reckless and tries to kill Noel as quickly as possible, for a moment, [[HopeSpot it looks as if he actually manages it]]. Then it turns out that was an illusion, and in reality, he had already been absorbed into Noel's mind, where he is forced to watch his own body, now solely controlled by Roa, turn around and disect Ciel into pieces. As he breaks down in tears, Noel promises him she has much more in store for him, intending to force him to endlessly reexperience his own trauma, while she makes her own additions and changes to keep him surprised. * As revealed in Ciel’s route, there are apparently regular street punks working for Vlov who lure innocent people down to the basement and throw them in to be devoured by The Dead. Any hope of them simply doing his bidding for survive is dashed when they are described as cheering as the shuffling corpses tear apart many survivors. With all of the horrific rampages the Dead Apostles cause in the game, it’s a frightening reminder [[HumansAreTheRealMonsters of how easily we can become hideously cruel without any supernatural pushing]].

* In both Arcueid's and Ciel's routes, a giant ghoul called Metastasis appears to fight off Ciel. While the smaller Krake ghouls are freakish in their own right, Metastasis is a humanoid spider monstrosity with one giant arm and one limb made out of multiple arms, full-on spider face and large spiky spider legs growing from its back. Ciel gets some mental damage just looking at it, comparing it to a painting that sounds like ''Art/TheStarryNight''. And considering [[CosmicHorrorReveal what we know about Vincent van Gogh]] in Nasuverse, the comparison she is making sounds like it might as well be a Lovecraftian horror.

* Noel's massacre of Vlov's lesser vampire spawn in Ciel's route is simply awful. She lures most of them to an abandoned building with false promises of turning them human again, then traps them inside with anti-vampire wards and begins butchering them all one by one while they scream and plead and beg for mercy. Those who try to escape die in agony as the wards melt their bodies. Keep in mind that these are not simply mindless dead or ghouls - they are fully self-aware lesser vampires, most of them are teenagers, and several seem to genuinely not know what is going on. There's also an implication that some may not even have fully turned, meaning they might actually have been saved if Noel hadn't murdered them. Oh, and she does it all with a smile on her face.
