13* The first GameOver you can get: simply put, Shiki is attacked and eaten by Nvrsqr Chaos at night and he can't defend himself. And all because you choose not to help Arcueid in her quest to kill Roa. [[SarcasmMode Well, so much for free will]].
14* Bad End 9. If Shiki decides to rape Arcueid in the back alley, the next time they met, she can't control herself and sucks his blood till he dies.
15* Bad End 8. If Shiki chooses to go home when Arcueid doesn't show up at the arranged time at the park, he gradually starts losing his memories of her and returns back to his normal life. It seems fine, until you think if [[FridgeHorror Arcueid ever has a chance to defeat Roa in her weakened state...]]
16* The remake averts GoryDiscretionShot during the "17 parts" scene at the beginning and shows you the butchered chunks of Arcueid's corpse in a pool of blood, staring Shiki in the face.

19* Bad End 3. When the hotel is attacked by Chaos, if Ciel's affection isn't high enough, she never sees Shiki after killing Arcueid, so she never gives him a charm to help repel vampires. This ends with Shiki going out into the hallway, exploring the empty hotel, and being swallowed by random Chaos animals in the dark.
20* Bad End 12. A completely out of nowhere bad ending (caused by not taking the time to ask her questions of a serious nature) where Roa appears, reveals about SHIKI and his connection to Shiki and kills him so swiftly that even Shiki doesn't realize what just happened.
21* Shiki manages to kill Roa, but he transfers the burden of his madness to him, causing Shiki to have murderous thoughts and strange dreams. After he gets out of control to the point that he rapes Kohaku.

24* Bad End 18. A vampirized Satsuki bites Shiki so they could be together forever.
25* Bad End 20. If Shiki doesn't tell Ciel about his nightmares, he goes to check at the back alley and has hallucinations of himself killing girls similar to Akiha (which is caused by the psychic link with SHIKI). He is then killed by SHIKI in a whim. What makes it horrible is that Shiki is laughing madly because this proves that he wasn't the killer as he thought.
26* Bad End 22. During Shiki and Ciel's battle, if Shiki decides to kill her, he loses his sanity and becomes convinced that he is the killer. He then reverts to his Nanaya instincts.

28* Bad End 23. During Shiki and SHIKI's fight, he dies after being impaled by multiple swords caused by SHIKI's BloodMagic.
29* SHIKI's death. After Shiki defeats him, he struggles to go back to Akiha's side. Let's not forget that this is Inverted Akiha, who is nothing more than a mindless monster. After SHIKI gets to Akiha, she crushes his heart and mauls him to death.
30* Akiha's Normal Ending: Distant Reed Warbler. SHIKI is dead, but Akiha is a mindless monster that feeds regularly on Shiki's blood. Poor Shiki can do nothing but wait until she returns to her old self again.
31** Which mirrors the situation with SHIKI and how he was imprisoned by Makihisa until he could return back to normal. Which means that Akiha will stay in her demon form forever.

34* The entire middle part of Hisui's route sees the protagonist immobile, in bed, gibbering unintelligibly as he descends into several different kinds of madness simultaneously. You're playing a game from the perspective of a character who is desperately, vainly trying to cling onto the tattered wreckage of his personality; afraid to be awake for fear of what he'll do to the people around him; terrified of going to sleep because of the murderous dreams that invade his mind; all the while tormented by agonizing migraines and uncontrollable bouts of furious rage...
35** And the very worst part of this is when, locked in the throes of madness, you see Kohaku's true smile for the first time.
36** Which isn't helped by Kohaku masquerading as Hisui at the time.
37* Bad End 29: Satsujinki. Shiki, completely under the influence of Kohaku's drugs, wakes up next to a girl's corpse and sees a crouching SHIKI, who seems to be in pain. After a small confrontation, Shiki decides that he is the murderer and kills him in a particularly gross scene, as Shiki vomits and laughs at the same time. He decides that now that SHIKI is dead, he will be the murderer from now on, as Kohaku laughs in the background.

42* SHIKI's death. He runs into Akiha and Kohaku. Akiha kills him by trapping him in her hair and absorbing him, as the guy has no idea what's going on. And all the time Kohaku has a SlasherSmile pointed at him.
43* Bad End 33. After Vermillion Akiha seemingly kills Kohaku, Shiki loses it and both fight. If Shiki decides to kill her immediately, he reverts to his Nanaya mode.
44* Bad End 34. Shiki dies tangled in Vermillion Akiha's hair, exactly in the same way SHIKI died.

47* The manga, by virtue of being illustrated, shows off much better why Shiki's Mystic Eyes nearly drove him insane at first. Having the entire hospital, and especially all the people in it, covered with the lines makes them look both incredibly fragile and freaky as all hell.
48* Shortly after little Shiki and SHIKI become friends, SHIKI has outbursts due to the Tohno blood in front of him. Then, Shiki succumbs to his Nanaya impulse and tries to strangle SHIKI, as both kids struggle until they come back to their senses. Shiki is of course freaked out by the whole thing while SHIKI just laughs, because it always happens to him. [[TroublingUnchildlikeBehaviour Let's not forget that in that moment they were only 6 and 7 respectively.]]
49* Think for a second about Shiki's life. He lost his entire family on a night, was then adopted by the man who orchestrated the whole thing, befriends the son of said man only to find out that he is an unstable HalfHumanHybrid, is killed by said boy after being possesed by a vampire, he is revived thanks to said boy's sister, gets brainwashed to forget all his memories of said boy and gets the Mystic Eyes of Death Perception, which nearly drove him insane the first time. What a TraumaCongaLine.
50* Also, we get a depiction of what happened to Ciel after she revived from being killed by Arcueid while possessed by Roa. Basically, the Church attempting to 'correct' as what they saw as an unnatural body, had her killed at least six hundred times including such deaths as by drowning, burning and organ removal, the whole time as she begged them to stop, only quitting when they decided she'd be more useful as a Burial agent. Afterwards, Ciel is shown as completely broken and resigned.

53* Noel's massacre of Vlov's lesser vampire spawn in Ciel's route is simply awful. She lures most of them to an abandoned building with false promises of turning them human again, then traps them inside with anti-vampire wards and begins butchering them all one by one while they scream and plead and beg for mercy. Those who try to escape die in agony as the wards melt their bodies. Keep in mind that these are not simply mindless dead or ghouls - they are fully self-aware lesser vampires, most of them are teenagers, and several seem to genuinely not know what is going on. There's also an implication that some may not even have fully turned, meaning they might actually have been saved if Noel hadn't murdered them. Oh, and she does it all with a smile on her face.
54* As revealed in Ciel’s route, there are apparently regular street punks working for Vlov who lure innocent people down to the basement and throw them in to be devoured by The Dead. Any hope of them simply doing his bidding to survive is dashed when they are described as cheering as the shuffling corpses tear apart many survivors. With all of the horrific rampages the Dead Apostles cause in the game, it’s a frightening reminder [[HumansAreTheRealMonsters of how easily we can become hideously cruel without any supernatural pushing]].
55* In both Arcueid's and Ciel's routes, a giant ghoul called Metastasis appears to fight off Ciel. While the smaller Krake ghouls are freakish in their own right, Metastasis is a humanoid spider monstrosity with one giant arm and one limb made out of multiple arms, full-on spider face, and large spiky spider legs growing from its back. Ciel gets some mental damage just looking at it, comparing it to a painting that sounds like ''Art/TheStarryNight''. And considering [[CosmicHorrorReveal what we know about]] Creator/VincentVanGogh in Nasuverse, the comparison she is making sounds as it might as well be a Lovecraftian horror.
56* Shiki [[SanitySlippage losing his mind to Roa's influence]] is so much worse in the Remake.
57** For a start, it is even more drawn out and agonizing for Shiki, to the point where there are gaps in his memories where the reader legitimately wonders if Shiki had become a serial killer off-screen. The extent of his madness is even shown more vividly with the damages he does to his own room and his increasingly chaotic point of view.
58** Kohaku's role is incredibly unnerving, especially for those who knows her role in the original. This time around, Shiki (or Roa) notices that she is calm and unsurprised, like she had experience handling monsters in human flesh. And we know she does, because that is exactly what happened with SHIKI. Even as Shiki begins physically attacking her, her [[DissonantSerenity smile]] never waves, making one wonders how much active manipulation she took part in leading Shiki to this state.
59** The worst part is, this is not even as bad as it can get. If Shiki [[JumpingOffTheSlipperySlope helps Noel "punish" vampires]], [[InLoveWithYourCarnage much to her delight]], it is implied that his more violent mental state makes him even more susceptible to Roa's influence, and his madness during the same scene is even worse, leading to him actively killing people, which he would otherwise have been able to hold himself back from. The damages done to his mind becomes even worse, and he begins to hear distorted voices from people who may or may not be there. And the worst part is he kills Akiha and Hisui [[ThroughTheEyesOfMadness without realizing it for two days]], with Kohaku covering it up and manipulating him into thinking everything is still fine, and him eventually killing her too. It culminates with Shiki coming to his senses, finding Kohaku [[HopeSpot alive]], before realizing that it is her reanimated lifeless corpse, to hammer home his vampirism. He runs to find Akiha, [[DeathSeeker believing she would be able to kill him]], only to find her and Hisui's corpse. Broken beyond repair, Shiki flees the now devoid of life Tohno mansion [[LaughingMad laughing]].
60* If Shiki chooses to be reckless and tries to kill Noel as quickly as possible, for a moment, [[HopeSpot it looks as if he actually manages it]]. Then it turns out that was an illusion, and in reality, he had already been absorbed into Noel's mind, where he is forced to watch his own body, now solely controlled by Roa, turn around and disect Ciel into pieces. As he breaks down in tears, Noel promises him she has much more in store for him, intending to force him to endlessly reexperience his own trauma, while she makes her own additions and changes to keep him surprised. [[FateWorseThanDeath Allowing him to be killed by Ciel earlier would have been an infinitely kinder fate]].
